摘要
目的:基于"脾统血"理论讨论易医脐针对血瘀水停型肝硬化腹水患者肝功能及生活质量的影响.方法:66 例血瘀水停型肝硬化患者按随机数字表法分为治疗组和对照组,每组33 例.两组患者均采用基础治疗,治疗组加用易医脐针,1 次/d,每疗程10 d,治疗2 个疗程.观察2 组患者治疗前后肝功能(ALT、AST、TBil、Alb)、凝血四项(PT、APTT、FIB、TT)、中医证候评分、超声腹水评分、腹围、体质量及生活质量评分的变化.结果:治疗组患者治疗后ALT、TBil、Alb、PT、APTT、FIB、中医证候评分、超声腹水评分、腹围、体质量及生活质量评分改善程度优于对照组(P<0.05),两组患者AST、TT改善差异无统计学意义(P>0.05).结论:基础治疗联合易医脐针治疗能够更好地改善血瘀水停型肝硬化腹水患者肝脏功能,减少腹水量,缩小腹围,减轻腹胀等不适症状,提高患者生活质量.
Abstract
Objective:Discuss the impact of I-Ching navel acupuncture on liver function and quality of life in patients with blood stasis and water retention type cirrhotic ascites,based on the theory of"spleen controlling blood".Methods:According to the random number table method,66 patients with cirrhosis of the liver with blood stasis and water stagnation were equally divided into treatment group and control group.Both groups were treated with basic therapy,in which the treatment group was treated with I-Ching navel acupuncture once a day for 10 d per course,for two courses.The changes of liver function,four items of coagulation,TCM evidence score,ultrasound ascites score,abdominal circumference,weight and quality of life score of the patients in the two groups were observed before and after treatment.Results:The improvement of ALT,TBil,Alb,PT,APTT,FIB,TCM evidence score,ultrasound ascites score,abdominal circumference,weight and quality of life scores in the treatment group were better than those in the control group(P<0.05).There was no statistically difference in the improvement of AST and TT between the two groups(P>0.05).Conclusion:The combination of basic treatment with I-Ching navel acupuncture could improve liver function,reduce abdominal water volume,shrink abdominal circumference,reduce abdominal distension and other uncomfortable symptoms,and improve the quality of life of patients with cirrhotic ascites with blood stasis and water stagnation.
