Risk Stratification System of Perioperative Anticoagulant Bleeding in Patients with Paroxysmal Atrial Fibrillation Treated by Radiofrequency Ablation
Objective:The risk factors of perioperative anticoagulant bleeding in patients with paroxysmal atrial fibrillation treated by radiofrequency ablation were analyzed,and the risk stratification system was constructed.Methods:A total of 282 patients with paroxysmal atrial fibrillation were divided into the training set(188 cases)and the verification set(94 cases)according to the random number table method at a ratio of 2∶1.According to perioperative anticoagulant bleeding,the patients in the training set were divided into bleeding group(42 cases)and non bleeding group(146 cases).The clinical data ofe two groups were compared and univariate analysis was performed.Multivariate Logistic regression was used to analyze the risk factors of perioperative anticoagulant bleeding.The receiver operating characteristic(ROC)curve and calibration curve were used to verify the nomogram model.The patient score was predicted according to the nomogram model,and the risk stratification system was established and verified with different HAS-BLED scores.Results:Hyperlipidemia,peripheral atherosclerosis,atrial fibrillation thrombosis risk(CHA2 DS2-VASc)score,HAS-BLED score,venous wall injury,anticoagulation interruption,and coagulation indicators were independent risk factors for perioperative anticoagulant bleeding(P<0.05).ROC curve and calibration curve results of training set and verification set showed that the nomogram model showed significant differentiation and accuracy.The hazard stratification system could well differentiated.Conclusion:Hyperlipidemia,peripheral atherosclerosis,CHA2 DS2-VASc score,HAS-BLED score,venous wall injury,anticoagulation interruption,and coagulation indicators are the risk factors for perioperative anticoagulant bleeding in patients with paroxysmic atrial fibrillation treated by radiofofency ablation.Hazard stratification system can predict the possihility of bleeding in patients.
