Correlation between Non-traditional Lipid Parameters and Type 2 Diabetes Mellitus Complicated with Coronary Heart Disease

目的:通过检测 2型糖尿病(T2DM)与合并冠心病(CHD)病人的非传统脂质指标,寻找预测T2DM合并CHD风险更灵敏的脂质参数,为临床预测 T2DM病人并发 CHD提供更有力的参数指标.方法:收集 2022 年 6 月—12 月于山西白求恩医院内分泌科及心血管内科住院的 T2DM病人 303例,根据冠状动脉造影结果分为两组,即单纯 T2DM组(150 例)和 T2DM合并 CHD组(153 例).记录两组一般临床资料及实验室指标等,同时根据传统血脂参数计算非传统脂质参数.采用 Logistic回归分析及受试者工作特征(ROC)曲线评价非传统脂质参数与 T2DM合并 CHD的相关性.结果:与单纯 T2DM组相比,T2DM合并 CHD组病人传统血脂参数高密度脂蛋白胆固醇(HDL-C)水平较低(P<0.05).然而,非传统脂质参数如总胆固醇(TC)/HDL-C、低密度脂蛋白胆固醇(LDL-C)/HDL-C、血浆致动脉粥样硬化指数(AIP)在 T2DM合并 CHD组均高于单纯 T2DM组(P<0.05).单因素 Logistic回归分析显示,HDL-C为T2DM合并 CHD的保护因素(OR=0.191),而非传统脂质参数 LDL-C/HDL-C(OR=1.376)、AIP(OR=2.215)、脂蛋白联合指数(LCI)(OR=1.009)为 T2DM合并 CHD的危险因素.进一步行多因素Logistic回归分析显示,HDL-C为T2DM合并CHD的独立保护因素(OR=0.077),而非传统脂质参数 LCI为 T2DM合并 CHD的危险因素(OR=1.023).ROC曲线显示,HDL-C、TC/HDL-C、LDL-C/HDL-C、AIP的 ROC曲线下面积(AUC)分别为 0.386,0.601,0.578,0.572,其中非传统脂质指标预测能力强于传统脂质.同时,LCI联合单因素 Logistic回归分析中差异有统计学意义的 HDL-C、LDL-C/HDL-C、AIP进行检测时 AUC则增大,说明传统 HDL-C和非传统脂质参数联合检测进一步提高了 T2DM合并 CHD的诊断效能.结论:HDL-C、LDL-C/HDL-C、AIP与 LCI为 T2DM合并 CHD的影响因素.LCI为 T2DM并发 CHD的危险因素.非传统脂质指标对 T2DM病人发生CHD的预测能力强于传统血脂参数,同时通过传统脂质参数HDL-C和非传统脂质参数的联合检测进一步提高了 T2DM合并 CHD的诊断效能.
Objective:To find lipid parameters that were more sensitive for predicting the risk of type 2 diabetes(T2DM)complicated with coronary heart disease(CHD),so as to provide more powerful parameters and theoretical support for clinical prediction of CHD in patients with T2DM.Methods:A total of 303 patients with T2DM were enrolled and divided into two groups according to the results of coronary angiography:T2DM alone group(150 cases)and T2DM combined with CHD group(153 cases).The general clinical data and laboratory indicators of the two groups were recorded,and the non-traditional lipid parameters were calculated according to the traditional blood lipid parameters.The correlation between non-traditional lipid parameters and T2DM combined with CHD was evaluated by Logistic regression analysis.Results:Compared with the T2DM group alone,the traditional blood lipid parameter HDL-C level in the T2DM complicated with CHD group was lower(P<0.05).However,non-traditional lipid parameters,such as TC/HDL-C,LDL-C/HDL-C,and AIP were higher in the T2DM and CHD group than that in the T2DM group alone(P<0.05).In univariate Logistic regression analysis,HDL-C was the protective factor for T2DM complicated with CHD(OR=0.191),while the traditional lipid parameters LDL-C/HDL-C(OR=1.376),AIP(OR=2.215)and LCI(OR=1.009)were the risk factors for T2DM complicated with CHD.Further multivariate Logistic regression analysis showed that HDL-C was an independent protective factor for T2DM and CHD(OR=0.077),while the non-traditional lipid parameter LCI was independently positively correlated with T2DM and CHD(OR=1.023),which was an independent risk factor for T2DM and CHD.The receiver operator characteristic(ROC)curves showed that the area under the cure(AUC)of HDL-C,TC/HDL-C,LDL-C/HDL-C,and AIP were 0.386,0.601,0.578,and 0.572,respectively,and the prediction ability of non-traditional lipid indexes was stronger than that of traditional lipids.At the same time,the AUC of HDL-C,LDL-C/HDL-C,and AIP with differences in LCI combined with univariate Logistic regression analysis increased,indicating that the combined detection of traditional HDL-C and non-traditional lipid parameters further improved the diagnostic performance of T2DM complicated with CHD.Conclusion:HDL-C,LDL-C/HDL-C,AIP,and LCI is the influencing factors of T2DM complicated with CHD.LCI is positively correlated with T2DM concurrent CHD.The predictive ability of non-traditional lipid indexes for CHD in patients with T2DM is stronger than that of traditional blood lipid parameters,and the combined detection of traditional HDL-C and non-traditional lipid parameters will further improve the diagnostic efficiency of T2DM complicated with CHD.
