摘要
目的 运用数据挖掘技术探讨小儿推拿治疗积滞的选穴规律.方法 通过Note Express软件检索中国期刊全文数据库(CNKI)、万方数据知识服务平台(Wanfang)和维普中文科技期刊数据库(VIP)建库至2022年10月31日小儿推拿治疗积滞的临床研究文献,采用Microsoft Excel 2010软件建立选穴处方数据库,统计腧穴频数及频率使用情况,并采用SPSS Modeler 18.0软件对处方进行关联规则分析,采用SPSS 22.0软件进行聚类分析.结果 共纳入108篇文献,包含116条推拿处方,涉及58个腧穴,腧穴使用总频数为777次.脾经、脊柱、腹、板门(四横纹)、中脘为小儿推拿治疗积滞使用频数前5位的腧穴,所用腧穴多分布于上肢部,脾经-四横纹、板门-脾经和腹-脾经为小儿推拿治疗积滞的高频穴对.结论 小儿推拿治疗积滞多为脏腑辨证选穴和局部选穴,核心腧穴为脾经、脊柱、腹、板门、四横纹、中脘,并根据临床辨证,灵活加减腧穴组合,以消食导滞、健脾理气,达到治疗效果.
Abstract
Objective To explore the pattern of acupuncture point selection in the treatment of pediatric stag-nation with tuina based on data mining.Methods The clinical research literature of tuina for the treatment of pedi-atric stagnation was searched by Note Express software from CNKI,Wanfang and VIP from the date of library con-struction to October 31,2022.Microsoft Excel 2010 software was used to establish a database of selected prescrip-tions,and the frequency of occurrence and use of acupoints were counted,and SPSS Modeler 18.0 software was used to analyze the prescriptions by association rules and SPSS 22.0 software was used to perform cluster analysis.Results Duplicate imported papers were excluded by using Note Express software,a total of 108 literatures containing 116 tuina prescriptions involving 58 acupoints were included,and the total frequency of acupoint use was 777.Spleen meridian,spinal column,abdomen,banmen(four transverse lines),and zhongwan were the top 5 acupoints used frequently in tuina for the treatment of pediatric stagnation,and the acupoints used were mostly distributed in the upper extremities,with spleen meridian-four transverse lines,banmen-spleen meridian,and abdomen-spleen meridi-an being the high-frequency pairs of acupoints used in massage for pediatric stagnation.Conclusion Most of the acupoints used in clinical massage treatment of pediatric stagnation were selected by internal organs identification and local selection,and the core acupoints were spleen meridian,spine,abdomen,banmen,four transverse lines,and zhongwan,and the combination of acupoints was added and subtracted flexibly according to clinical identifica-tion to eliminate food and guide stagnation and to strengthen the spleen and regulate qi to achieve therapeutic effects.
