推拿联合针刀治疗儿童股骨骨髓炎后膝关节挛缩1例报告

摘要
记录1例股骨骨髓炎患儿的发病过程、诊断依据及西医抗感染治疗疗程;详细介绍患儿股骨骨髓炎后膝关节挛缩的临床表现;具体叙述推拿联合针刀治疗小儿股骨骨髓炎后膝关节挛缩的手法操作部位、时间、注意事项及治疗疗程;回顾分析推拿手法操作及针刀治疗的理论基础;从经络腧穴入手,结合现代膝关节解剖知识,采用推拿手法作用于患肢的肌肉、关节,联合针刀于左下肢半腱半膜肌、跟腱处予以松解.该例患儿经过针刀治疗10次结合推拿治疗4个月后,局部肌肉痉挛、肿胀得到有效缓解,关节活动度得以明显改善,患肢肌力明显增加,膝关节屈曲明显改善,胭窝角可达150°,可独走,关节功能得到较大程度的恢复.
Abstract
This paper records the pathogenesis,diagnostic basis and course of anti-infection treatment in West-ern medicine of a child with femoral osteomyelitis,and describes in detail the clinical manifestations of knee joint contracture after osteomyelitis in the child.This paper describes the operation site,time,precautions and treatment course of tuina and needle knife for the treatment of knee joint contracture after osteomyelitis of the femur;reviews and analyses the theoretical basis of tuina and needle knife treatment;starts from the meridian acupoints and com-bines with the modern knowledge of knee joint anatomy,adopts tuina to act on the affected limb's muscles and joints,and combines with the needle knife in the lower limb of the left semitendinosus semimembranosus muscle and achilles tendon to be released,the case has been treated with needle knife.After 10 times of needle knife treatment combined with tuina therapy for 4 months,the local muscle spasm and swelling were effectively relieved,the joint mobility was significantly improved,the muscle strength of the affected limb was significantly increased,the knee flexion was significantly improved,the popliteal fossa angle could reach 150 °,and the child could walk unaided,and the function of the joints was restored to a greater extent.
