Network Meta-analysis of treating insomnia after stroke by acupuncture
Objective:To systematically evaluate the efficacy and safety of different acupuncture in the treatment of insomnia after stroke.Methods:PubMed,CNKI and other Chinese and English databases were searched by computer to collect the randomized controlled trial of acupuncture in the treatment of insomnia after stroke published from 2010 to March,2022.The included data were analyzed by Revman 5.3 and Stata 16.0 software.Results:A total of 39 literatures were included,including 2 551 patients,involving 9 acupuncture methods,including routine acupuncture,ear acupuncture,head governor vein arrangement acupuncture,acupoint catgut embedding,scalp acupuncture,wrist ankle acupuncture,eye acupuncture,acupuncture combined with acupoint catgut embedding,acupuncture combined with ear acupuncture.The results of net Meta-analysis showed that ① in terms of the effective rate of sleep improvement,the cumulative probability of the area under the surface under the cumulative ranking curve(SUCRA)was acupuncture combined with acupoint catgut embedding>acupoint application>head governor vein arrangement acupuncture>wrist ankle acupuncture>scalp acupuncture>ear acupuncture>acupoint catgut embedding>acupuncture>acupuncture combined with ear acupuncture.② In terms of Pittsburgh sleep index(PSQI)score,the cumulative probability of SUCRA is acupuncture combined acupoint catgut embedding>head governor vein puncture>eye acupuncture>acupuncture>head acupuncture.In terms of safety,the incidence of adverse reactions/events in the experimental group was lower than that in the control group.Conclusion:Acupuncture combined with acupoint catgut embedding can significantly improve the clinical efficacy on post-stroke insomnia.However,in view of the large differences in the number of studies and methodological quality between different treatment measures,the SUCRA cumulative probability ranking results of acupuncture need to be verified by multi center,large sample,randomized double-blind clinical controlled trials in the future.
