目的:研究肾衰解毒汤联合血液透析治疗慢性肾功能衰竭(Chronic Renal Failure,CRF)的临床疗效及对患者血清同型半胱氨酸的影响,以期为CRF的临床治疗提供参考.方法:选取2018年1月-2019年12月江苏省邳州市中医院血液透析中心收治的60例CRF患者作为研究对象,按随机数字表法分为研究组与对照组,每组30例,对照组给予常规血液透析治疗,研究组在常规血液透析的基础上联合使用肾衰解毒汤浓煎液口服,两组均治疗1个月.比较两组患者治疗前后血清同型半胱氨酸(Homocysteine,Hcy)、β2-微球蛋白(β2-microglobulin,β2-MG)、血尿素氮(Blood Urea Nitrogen,BUN)、血肌酐(Serum Creatinine,Scr)、尿白蛋白排泄率(Urinary Albumin Excretion Rates,UAER)水平及治疗期间的不良事件.结果:治疗前两组Hcy、β2-MG、BUN、Scr、UAER差异均无统计学意义(P＞0.05),治疗后研究组Hcy、β2-MG、BUN、Scr、UAER低于对照组,差异有统计学意义(P＜0.05).治疗后研究组低血压、低氧血症、总不良反应率低于对照组,差异有统计学意义(P＜0.05).结论:肾衰解毒汤具有健脾补肾、益气养血、泄浊解毒之效,联合血液透析可有效改善CRF患者的临床症状,增强患者的肾小球滤过功能,改善肾功能,改善血液及尿液相关指标的表达水平,减少患者血液透析过程中低血压、低氧血症等并发症的发生概率,从而降低患者的总体透析风险.该方法疗效确切且安全性高、经济适用性强,可减轻患者的经济负担,具有临床推广价值.
Clinical observation of Shenshuai Jiedu decoction combined with hemodialysis in the treatment of chronic renal failure and its effects on serum homocysteine
Objective:To study the clinical efficacy of Shenshuai Jiedu decoction(肾衰解毒汤)combined with hemodialysis in the treatment of chronic renal failure and its effects on serum homocysteine,in order to provide reference for the clinical treatment of chronic renal failure.Methods:A total of 60 patients with chronic renal failure admitted to the Hemodialysis Center of Pizhou Hospital of Traditional Chinese Medicine in Jiangsu Province from January 2018 to December 2019 were selected as research subjects and divided into the study group and the control group according to random number table method,with 30 cases in each group.The control group was given routine hemodialysis treatment,while the study group was given oral administration of Shenshuai Jiedu decoction on the basis of routine hemodialysis.Both groups were treated for one month.The levels of Hcy,β2-MG,BUN,Scr and UAER before and after treatment and the adverse events during treatment were compared between the two groups.Results:There was no significant difference in Hcy,β2-MG,BUN,Scr and UAER between the two groups before treatment(P＞0.05).After treatment,Hcy,β2-MG,BUN,Scr and UAER in the study group were lower than those in the control group,and the difference was statistically significant(P＜0.05).After treatment,the rate of hypotension,hypoxemia and total adverse reactions in the study group was lower than that in the control group,and the difference was statistically significant(P＜0.05).Conclusion:Shenshuai Jiedu decoction has the effects of invigorating spleen and kidney,invigorating Qi(气)and nourishing blood,and relieving turbidity and detoxification.Combined with hemodialysis,it can effectively relieve the clinical symptoms of patients with chronic renal failure,enhance the glomerular filtration function of patients,improve kidney function,and the expression levels of blood and urine related indicators,and reduce the probability of complications such as hypotension and hypoxemia in the process of hemodialysis.Therefore,the overall dialysis risk of patients can be reduced.This method is effective,safe and economical,can reduce the economic burden of patients,and has clinical popularization value.
