Professor DONG Rui's experience in treating pulmonary nodules
Pulmonary nodules are regular or irregular lesions with a diameter of less than 3 cm in lung tissue,which can be single or multiple,and the boundary is clear or unclear.According to the characteristics of lesion density,pulmonary nodules were divided into three types:ground glass nodules,partial solid nodules and solid nodules.Among them,the malignant degree of partial solid nodules was the highest,followed by ground glass nodules and solid nodules.With the popularization of chest CT,the incidence of pulmonary nodules has increased significantly in recent years.From the perspective of Western medicine,pulmonary nodules can only be observed regularly,and when the nodule enlarges in a short period of time or surgery is indicated,surgery is recommended.Traditional Chinese medicine advocates active conditioning,syndrome differentiation and treatment,regulating emotions,and improving physical fitness,which are beneficial to the long-term prognosis of pulmonary nodules.Professor DONG Rui believes that pulmonary nodule belongs to the category of pulmonary accumulation and accumulation in TCM,with asthenia in origin and excess in superficiality.The origin is deficiency of vital Qi(气),deficiency of spleen and kidney Yang(阳),and the superficial excess is that phlegm and blood stasis are intertwined and damage lung collaterals.Generally,the disease was seen in patients with Qi deficiency,Yang deficiency,phlegm dampness,blood stasis or Qi stagnation constitution type.A new idea of regulating pulmonary nodules from physique theory was put forward.The treatment is warming Yang,nourishing Yang,tonifying lung and dispersing lung,warming kidney,dispelling cold,promoting blood circulation,dredging collaterals,softening and resolving hard masses.The pathogenesis of pulmonary nodules is complex,the physique of the patients is different,and the treatment needs a long course of treatment,which is suitable for the comprehensive therapy of large prescription.The main syndromes are spleen and kidney Yang deficiency and phlegm and blood stasis blocking collaterals.The Kangyi Wenyang Huajie ointment(康益温阳化结膏)and DONG's Jinjia powder(董氏金甲散)are established and used in clinical application,can effectively control the growth rate of pulmonary nodules,and reduce the volume of pulmonary nodules.
