Diabetic nephropathy(DN)refers to kidney damage caused by long-term diabetes.Elevated blood glucose levels in patients lead to renal impairment,resulting in clinical symptoms primarily characterized by proteinuria.Western medicine lacks specific drugs for DN,and conventional treatments focus on controlling blood glucose and blood pressure,as well as using drugs such as angiotensin-converting enzyme inhibitors to reduce proteinuria and slow down the progression of DN.However,these treatments have limited targets and often come with adverse reactions.In end-stage of renal disease,altemative therapies such as dialysis and kidney transplantation impose significant financial burdens on patients,leading to treatment dilemmas.In contrast,TCM has a long history in the treatment of DN.TCM possesses unique advantages due to its multi-target,multi-component,and multi-perspective characteristics.TCM classifies DN as kidney deficiency and edema based on symptoms.This article explores the TCM etiology and pathogenesis of DN from a TCM perspective,collecting clinical and relevant mechanistic research articles on TCM compound formulas,single herbal medicines,and active ingredients for the treatment of DN.The article reviews TCM compound formulas,single herbal medicines,and active ingredients separately.The relevant literature on the treatment of DN with TCM compound formulas is analyzed from the perspectives of promoting blood circulation and removing blood stasis,regulating Qi(气)and promoting water metabolism,clearing heat and eliminating dampness,as well as other treatment methods.Single herbal medicines for DN are discussed from the perspectives of nourishing deficiency,heat-clearing,and promoting blood circulation and resolving stasis.The treatment of DN using individual TCM active ingredients is categorized based on their pharmacological structures.The clinical efficacy,relevant pharmacology,and mechanism research of these three treatment approaches are explored.The article summarizes the theoretical evidence,advantages,and limitations of TCM in the treatment of DN,aiming to provide new perspectives for the development of preventive and therapeutic drugs for DN.
