A review on the mechanism of blood-brain barrier injury in chronic cerebral ischemia and its TCM intervention measures
The blood-brain barrier is located between the blood and the neurons in the brain and spinal cord.It is the physical and biochemical barrier of material exchange between the central nervous system and the circulatory system,and is of great significance to maintain the stability of the internal environment and normal physiological function of the central nervous system.Chronic cerebral ischemia is a reversible ischemic cerebrovascular disease caused by decreased cerebral blood perfusion.Chronic cerebral ischemia can activate a cascade of molecular and cellular injury,thereby affecting the components and structure of the blood-brain barrier and impairs its integrity.The disease is more common in middle-aged and elderly people.Vertigo,sleep disturbance and cognitive impairment are the most common clinical manifestations of the disease.Without timely diagnosis and treatment,the disease can progress to white matter lesions,Alzheimer's disease,cerebral small vessel disease,acute stroke and other diseases,seriously affecting the physical and mental health of patients.In this paper,the relevant literature on the mechanism of blood-brain barrier injury caused by chronic cerebral ischemia and its TCM intervention measures in recent ten years are retrieved and briefly summarized from the perspective of cellular and molecular mechanisms.It is found that the destruction of blood-brain barrier is an important pathological change of chronic cerebral ischemia.Specifically,cerebral hypoperfusion caused by long-term ischemia and hypoxia can induce a series of pathophysiological changes such as apoptosis,immune inflammatory injury,vascular endothelial dysfunction,which further aggravate blood-brain barrier injury.TCM treatment takes the holistic concept as the core thinking mode.Experimental and clinical studies have confirmed that TCM can relieve the symptoms related to chronic cerebral ischemia such as dizziness and insomnia,repair the blood-brain barrier,reduce the degree and extent of blood-brain barrier injury,and had feasibility of delaying the occurrence and progress of the above diseases.TCM has the advantages of multi-target and multi-pathway treatment,and has broad clinical application prospects.
