脑小血管病与失眠关联性的研究进展

摘要
脑小血管病会导致失眠的发生,而长期慢性失眠又是脑小血管病的高危因素之一,两者关系紧密,互为因果.现代研究表明,以脑小血管病的影像学分型、发病机制为依据,腔隙性脑梗死、脑白质高信号、脑微出血、血管周围间隙扩大及脑萎缩均与失眠有较强的关联性.中医研究表明,脑小血管病多属络病,气虚血瘀是脑小血管病伴失眠的关键病机.临床治疗脑小血管病伴失眠多以中西医结合治疗及非药物疗法并用,优势在于运用西药迅速缓解症状,预防、控制疾病的危险因素,运用中药及非药物疗法整体调节人体功能,使之达到阴阳平衡的状态.文章对脑小血管病与失眠关联性的研究进展进行综述,以期为临床诊治提供借鉴意义.
Abstract
Cerebral small vessel disease will lead to insomnia disorder,and long-term chronic insomnia is one of the high risk factors of cerebral small vessel disease.The two are closely related and cause and effect each other.Modern studies have shown that based on the imaging classification and pathogenesis of cerebral small vessel disease,lacunar infarction,white matter hyperintensities,cerebral microbleed,enlarged perivascular spaces and brain atrophy are all strongly associated with insomnia.Traditional Chinese medicine research shows that cerebral small vessel disease mostly belongs to collateral disease,and Qi(气)deficiency and blood stasis is the key pathogenesis of cerebral small vessel disease with insomnia.The clinical treatment of cerebral small vessel disease with insomnia is mainly treated by the combination of traditional Chinese and western medicine and non-drug therapy,the advantage lies in the use of western medicine to quickly relieve symptoms and prevent and control the risk factors of the disease,the use of traditional Chinese medicine and non-drug therapy to regulate human function as a whole to achieve the balance of Yin(阴)and Yang(阳).This article reviews the research progress on the relationship between cerebral small vessel disease and insomnia,in order to provide reference for clinical diagnosis and treatment.
