The traditional Chinese medicine strategy of treating vaginitis with the Gancao Xiexin decoction and its impact on vaginal microecological balance based on the real world
Objective:Based on the real world observation on the clinical efficacy of the Gancao Xiexin decoction(甘草泻心汤)on various types of vaginitis and vaginal microecological balance,to develop the corresponding TCM clinical treatment strategy.Methods:Ninety patients with vaginitis were selected and divided into three groups:non-exposure group,low-exposure group and high-exposure group based on prospective cohort study.The non-exposure group received conventional Western medicine according to clinical guidelines,while the exposed groups received oral treatment with the Gancao Xiexin decoction alone or the Gancao Xiexin decoction on the basis of conventional Western medicine.The low-exposure group took the Gancao Xiexin decoction for more than 2 weeks but less than 1 month,and the high-exposure group took the Gancao Xiexin decoction for more than 1 month.All three groups were followed up for 6 months.The improvement time of clinical symptoms and signs,clinical total effective rate and recurrence rate were compared among the three groups.The clinical symptom score,TCM syndrome score and vaginal microecological environment indexes were compared before and after treatment.Results:The Gancao Xiexin decoction could effectively improve the common clinical symptoms and signs of vaginitis,such as vulva pruritus,abnormal vaginal secretions,vaginal congestion,etc.,and with the extension of intervention time,the improvement of various indexes was better(P＜0.05).After intervention for more than one month,the Gancao Xiexin decoction could effectively and stably reduce clinical symptom score,TCM syndrome score and improve various indexes of vaginal microecological environment(P＜0.05).The total effective rate in the non-exposure group,low-exposure group and high-exposure group was 33.33％(4/12),70.83％(17/24)and 94.44％(51/54),respectively(P＜0.05).After 6 months of followed-up,the recurrence rate in the exposed groups were significantly lower than that in the non-exposure group,and with the extension of intervention time,the recurrence rate was further reduced(P＜0.05).Conclusion:The application of the Gancao Xiexin decoction for more than one month can effectively improve the clinical effective rate of various types of vaginitis,reduce the recurrence rate,relieve various clinical symptoms and signs,and regulate the vaginal microecological environment.
