目的:通过Meta分析对耳穴疗法治疗多囊卵巢综合征(Polycystic Ovary Syndrome,PCOS)患者肥胖疗效进行评价,探讨耳穴疗法治疗PCOS患者肥胖症状的临床价值,以期为临床治疗PCOS提供循证护理证据.方法;检索PubMed、Cochrane Library、Web of Science、Embase、中国知网、万方数据、维普(中文科技期刊数据库)和中国生物医学文献服务系统中关于耳穴疗法治疗PCOS患者肥胖症状的随机对照试验(Randomized Controlled Trial,RCT)或类实验研究,检索时限为建库起至2023年3月1日,采用软件RevMan 5.4进行分析.结果:最终纳入13篇文献,共计包含866例PCOS患者.Meta分析结果显示,试验组(耳穴组)PCOS患者治疗有效率高于对照组[比值比(OR)=2.37,95％置信区间(CI)(1.59,3.54),P＜0.000 1];耳穴组体质量指数(Body Mass Index,BMI)低于对照组[MD=-1.16,95％CI(-2.15,-0.17),P＜0.05];耳穴组中医证候积分低于对照组[MD=-1.65,95％CI(-2.40,-0.91),P＜0.000 1].结论:耳穴疗法可以改善PCOS患者体质量及中医临床症状,有利于提高治疗有效率,且临床表明采用耳穴疗法联合其他中医技术或中药汤剂效果更好.但考虑到纳入研究数量不多,且均为中文文献,有方法学质量限制且结局指标较少,较为单一,Meta分析的规模小,建议在未来研究中增加结局指标,期待未来临床开展更多大样本、高质量的RCT研究,为耳穴疗法治疗PCOS患者肥胖症状提供更有力的临床证据.
A Meta-analysis of the effect of auricular acupoint therapy on obesity in patients with polycystic ovary syndrome
Objective:To evaluate the effect of auricular acupoint therapy on obesity in patients with polycystic ovary syndrome(PCOS)through Meta-analysis,and to explore the clinical value of auricular acupoint therapy on obesity in patients with PCOS,in order to provide evidence-based nursing evidence for clinical treatment of PCOS.Methods:PubMed,Cochrane Library,Web of Science,Embase,CNKI,Wanfang Data,VIP(Chinese Science and Technology Journal Database)and Chinese biomedical Literature database were searched for randomized controlled trials on auricular acupoint therapy in the treatment of obesity in patients with PCOS from the establishment of the database to March 1,2023.Revman 5.4 software was used to analyze the data.Results:Finally,a total of 13 studies involving 866 patients with PCOS were included.Meta-analysis showed that the effective rate in the auricular acupoint group was higher than that in the control group[OR=2.37,95％CI(1.59,3.54),P＜0.000 1].The BMI in the auricular acupoint group was lower than in the control group[MD=-1.16,95％CI(-2.15,-0.17),P＜0.05].The TCM syndrome score of the auricular acupoint group was lower than in the control group[MD=-1.65,95％CI(-2.40,-0.91),P＜0.000 1].Conclusion:Auricular acupoint therapy can improve the body weight and relieve the clinical symptoms of patients with PCOS,and is beneficial to improve the treatment efficiency.Clinical results show that auricular acupoint therapy combined with other traditional Chinese medicine techniques or traditional Chinese medicine decoction is better.However,considering the small number of included studies,all of which are in Chinese,with methodological quality limitations,and fewer and relatively single outcome indicators,it is suggested to add outcome indicators in the future studies,and more large-sample and high-quality randomized controlled trials are expected to be carried out in the future to provide stronger clinical evidence for auricular therapy in the treatment of obesity symptoms in patients with POCS.
