A brief analysis of pulmonary interstitial fibrosis from the ascending,descending,exiting and entering theory in Duyi Suibi
Pulmonary interstitial fibrosis(IPF)is a chronic diffuse interstitial lung disease characterized by lung fibroblasts and muscle fibroblasts abnormal proliferation,and extracellular matrix proteins excessive deposition and abnormal regulation,which can lead to impaired gas exchange,dyspnea,decreased quality of life and even death due to respiratory failure of patients.Its pathogenesis is complex.According to the clinical characteristics,IPF belongs to the categories of lung palsy and lung flaccidity in traditional Chinese medicine.Based on the Xuanfu(玄 府)theory and Zang-fu viscera(脏腑)theory in TCM,this paper began with the ascending,descending,exiting and entering theory in Duyi Suibi(《读医随笔》)by ZHOU Xuehai,a famous TCM practitioner in the Qing Dynasty,and summarized the basic pathogenesis of IPF as deficiency in origin and excess in superficiality,taking lung Qi(气)deficiency as the foundation and phlegm and blood stasis coagulation as exterior symptoms.After a long illness,Qi is comsumed,and heat is endogenous,which affects the ascending,descending,exiting,and entering of Qi movement,leading to the production of phlegm and blood stasis.The author discusses the relationship between IPF and the ascending,descending,exiting and entering of Qi movement of human body,and believes that the adjustment of Qi movement of human body is beneficial to alleviate the symptoms of IPF,and discusses the treatment from the aspects of opening Xuanfu,soothing liver and regulating lung,nourishing spleen and stomach,and dispelling phlegm and removing blood stasis,in order to analyze the diagnosis and treatment of IPF in TCM.
