摘要
间质性肺病(Interstitial Lung Disease,ILD)病程较长,因阶段不同而症状表现各异.张常喜主任认为该病按病理阶段的不同可诊断为两个病,疾病早期主要为外邪干肺、气血痹阻,故表现为咳嗽、胸闷,宜诊断为肺痹;随着外邪痹阻日久致气阴虚损,又因劳役导致大汗,而使得津液亏虚,肺失濡养,不能宣通诸脏之气,致肺叶萎弱不用时,可诊断为肺痿.因此,张主任提出应"分段论治".但在分段论治时也需注重"急则治其标,缓则治其本"的原则,在疾病的不同病理阶段也各有侧重,治标时注重祛瘀化痰、活血通络,治本时注重调补肺肾、益气健脾.根据疾病不同阶段的不同特点,做到辨病与辨证结合,中医与西医结合优化治疗.ILD属于难治性疾病,中医治疗辨证施治,不仅可以改善患者症状,提高临床疗效,而且可调节机体免疫功能.
Abstract
The course of interstitial lung disease is long,and the symptoms vary according to the different stages of the disease development.Director ZHANG Changxi believes that the disease should be diagnosed as two diseases according to the different pathological stages.In the early stage of the disease,it should be diagnosed as lung arthralgia mainly due to extemal pathogenic invades lung,Qi(气)and blood ob-struction,which manifests as cough and chest tightness.And with the obstruction of pathogenic factors in the lung for a long time,it causes Qi and Yin(阴)deficiency,and combines with sweating because of labor,which leads to the body fluid deficiency,lung fails to be nourished,which in tum can not promote the Qi of the viscera,and when the lung is weak and not used,it can be called lung impotence.Therefore,in the course of treating this disease,Director ZHANG proposes"Staged treatment".At the same time,attention should also paid to the principle of"Urgent treatment of the symptoms and slow treatment of its root causes",the different pathological stages of the disease also have their own focuses,in the treatment of symptoms,the focus is on removing extemal pathogenic factors,at the same time,the treatment should also remove blood stasis and phlegm,promoting blood circulation,in the treatment of the root causes,attention should be given to tonify lung and kidney,Qi and spleen.According to the different characteristics of different stages of the disease,the combination of disease differentiation and syndrome differentiation,the combination of traditional Chinese medicine and Westem med-icine,and the optimization of treatment according to their own characteristics should be achieved.Interstitial lung disease is a refractory disease.The use of syndrome differentiation in TCM treatment can not only improve the quality of life of patients,but also relieve the symptoms of patients.Traditional Chinese medicine has obvious advantages in the treatment of interstittial lung disease,which can not only improve the clinical efficacy,but also regulate the collective immune function,and can significantly improve the quality of life of patients.
