浅谈吴瑭《温病条辨》甘味药治疗消渴病的运用

现代医学糖尿病归属于消渴病范畴.《黄帝内经》最早提出"消渴"病名,其对消渴病的病因认识主要涉及五脏柔弱、过食肥甘、情志失调、外感六淫等方面,历代医家在此基础上对消渴病的病因病机进行了不断探讨与深入.《温病条辨》是吴瑭(号鞠通)在总结《黄帝内经》《伤寒杂病论》理论、私淑叶桂(字天士)学术思想及结合临床经验的基础上所成的一部温病学专著,其开创了温病三焦辨治之先河,建立了一个完全创新且独立的温病学理论体系.吴瑭善于将药物的性味配伍理论应用于各类温病治疗中.其从三焦系统辨治消渴病,通过药物的性味配对应用于消渴病的临床治疗中,为后世提供了典范,值得进一步深入研究.甘味药在《温病条辨》中的应用有2个方面,就其单味药而言,具有补益和中、调和药性、缓急止痛等功效;就与其他药物性味配伍而言,可发挥甘寒生津、酸甘化阴、甘苦合化等功效.基于这些特点,吴瑭在《温病条辨》中对甘味药的运用十分广泛,且颇具特色.此研究从三焦辨治体系出发,探讨消渴病的病因病机,通过整理、归纳《温病条辨》中甘味药的运用,分析其在消渴病治疗中的性味配伍特点以及规律,以期为现代临床诊疗消渴病提供新的思路.
A brief discussion on the application of sweet medicinal in the treatment of diabetes in WU Tang's Item Differentiation of Warm Febrile Diseases
Xiaoke disease belongs to the category of diabetes in modem medicine.The Yellow Emperor's Canon of Internal Medicine is the first to propose the name"Xiaoke".Its understanding of the causes of diabetes mainly involves the weakness of the five internal organs,overeating fat and sweet food,emotional imbalance,and six climate exopathogens.On this basis,the ancient and modem medical practitioners have carried out continuous exploration and in-depth study on the etiology and pathogenesis of diabetes.Item Differentiation of Warm Febrile Diseases is a treatise on febrile diseases written by WU Tang after summarizing the theories of The Yellow Emperor's Canon of Internal Medicine and Treatise on Febrile Diseases and learning from YE Tianshi's academic thought and clinical experience.It pioneers the differentiation and treatment of febrile diseases through three Jiao and established a completely innovative and independent theoretical system of febrile diseases.WU Tang is good at applying the theory of compatibility of drug properties and tastes to the treatment of various febrile diseases.He distinguishes and treats diabetes from three Jiao system,and applies the compatibility of drug properties and tastes in the clinical treatment of diabetes,providing a model for future generations and worthy of further in-depth research.The application of sweet medicinal in Item Differentiation of Warm Febrile Diseases has the effect of supplementing and mediating the drug,alleviating the pain and so on as far as its single drug efficacy is concemed.In terms of the compatibility of sweet medicinal with properties and flavors of other medicinals,it can exert effects such as sweet and cold generating saliva,sour and sweet transforming Yin(阴),and sweet and bitter combining.Based on these characteristics,WU Tang's use of sweet medicinal in Item Differentiation of Warm Febrile Diseases is very extensive and distinctive.This article aims to explore the etiology and pathogenesis of diabetes from the perspective of three Jiao diagnosis and treatment system.By organizing and summarizing the use of sweet medicinal in Item Differentiation of Warm Febrile Diseases,the characteristics and laws of their compatibility in the treatment of diabetes are analyzed,in order to provide new ideas for modem clinical diagnosis and treatment of diabetes.
