Treating irritable bowel syndrome based on the Erdan theory
Irritable bowel syndrome(IBS)is a relatively common functional gastrointestinal disorder disease in clinical practice,mainly characterized by long-term abdominal pain,abdominal distention and abnormal bowel habits.The disease is prone to recurrence and lingering,which seriously affects quality of life of the patients.Due to the fast pace of modern life,the incidence rate of the disease is increasing year by year.Nowadays,many doctors believe that it is mainly induced by internal injury factors such as emotion,diet,etc.,and the idea of differentiation and treatment is based on this.However,many classical medical books clearly pointed out that external exogenous pathogens could cause the formation of diarrhea,abdominal pain and so on.This article takes this as a starting point to explore the correlation between external pathogenic factors and IBS,and treats the disease based on the pathogenesis.Based on the concepts of holistic view and treating different diseases by the same method in traditional Chinese medicine,the feasibility of the Erdan(二旦)theory in treating IBS is explored.The etiology,pathogenesis,course evolution,and prognosis of IBS are mostly caused by external pathogens entering the interior,resulting in abnormal vital energy and affecting the viscera.The core of the pathogenesis lies in the spleen and stomach,which is specifically manifested as the transformation of Qi(气)and body fluid.The pathogenesis is consistent with the syndrome and action mechanism of the Erdan method.Therefore,in terms of its treatment,the fundamental principle should be the Erdan principles of treating external pathogenic factors to promote the circulation of Yang(阳)to disperse body fluid,and nourish Yin(阴)to reduce turbidity,in order to obtain the harmony of Qi and body fluid of internal organs.Specifically,according to the differences in deficiency and excess on the surface and inside,the Erdan decoction(二旦汤)and its derivatives can be used to treat different types based on TCM differentiation,providing reference for the clinical treatment of IBS.
