Discussion on treating nasal vestibulitis by acupuncture based on the theory of treating orofacial disorders by stimulating the Hegu acupoint
Nasal vestibulitis is a prevalent disease in the field of otolaryngology,clinically manifested as diffuse redness and pain in the nasal vestibular skin,and is an undeniable public health concern.In modern medical treatment,topical corticosteroid ointments are primarily used,which have significant adverse reactions and can lead to dependence.Traditional Chinese medicine(TCM)primarily focuses on oral administration and external application of traditional Chinese medicines,which are less efficacious than acupuncture.As one of the external treatments in TCM,acupuncture is characterized by its simplicity,cost-effectiveness,and ability to effectively alleviate pain and improve symptoms of nasal vestibulitis.However,there are currently few clinical reports on treating this disease by acupuncture.In TCM,nasal vestibulitis is termed nasal sores,and it is believed to be associated with external pathogens invading or internal heat burning.The pathogenesis is attributed to lung heat ascending to the nose,which is related to Dachang(大肠)meridian of the Hand Yangming(阳明).Therefore,treatment starts from Dachang meridian,and the relationship between the lung and large intestine is used to treat the external orifices of the lung,while leveraging the meridian effect of"Wherever the meridian passes,that's where the governance reaches".Hegu acupoint(LI4)is the original point of Dachang meridian of the Hand Yangming,can clear heat from relevant organs,regulate the energy of the triple warmer and the original energy of the kidney,for harmonizing Qi(气)and blood throughout the body,drawing energy back to its original place,and stimulating the body's self-healing ability.Hegu acupoint is also adept at moving through the head and face,dispersing wind and cooling down heat,thereby reducing swelling and pain.Treating orofacial disorders by stimulating the Hegu acupoint in the Qiankun Shengyi(《乾坤生意》)is regarded as a fundamental principle by later generations of physicians.Modern research has found that there is a close relationship between Hegu area and the sensory and motor cortex distribution in the facial area of the cerebral cortex.This is closely related to the meridian theory of the Hand Yangming Dachang meridian.This article will explore the mechanism of acupuncture at Hegu acupoint in treating nasal vestibulitis based on the TCM theory and modern medicine,to provide a theoretical basis for clinical acupuncture treatment of nasal sores.Through case study,this article further elaborates on the experience,hoping to provide clinical insights for the treatment of nasal vestibulitis by acupuncture.
