目的:研究长桑君脉法脉息术指导下大柴胡汤联合针刺疗法对2型糖尿病患者的临床疗效，以期为临床治疗提供新思路。方法:运用长桑君脉法脉息术对醴陵市中医院2023年1月-12月收治的2型糖尿病患者进行脉息比检测，共筛选出60例脉躁型2型糖尿病患者，运用随机数字表法将其分为治疗组与对照组，每组30例。治疗组采用大柴胡汤联合针刺治疗，对照组予西医常规治疗。治疗后观察两组患者的血糖指标[空腹血糖(Glucose，GLU)、餐后2 h血糖(2-hour Postprandial Blood Glucose，2 h PBG)、糖化血红蛋白(Gycosylated Hemoglobin，HbA1 c)]水平、血脂指标[三酰甘油(Triglyceride，TG)、总胆固醇(Total cholesterol，TC)、低密度脂蛋白胆固醇(Low Density Lipoprotein Cholesterol，LDL-C)、高密度脂蛋白胆固醇(High Density Lipoprotein Cholesterol，HDL-C]水平、胰岛素抵抗情况、中医证候积分及临床疗效。结果:治疗8周后，两组患者的血糖相关指标(GLU、2 h PBG、HbA1 c)均较治疗前下降，差异有统计学意义(P＜0。05)，且治疗组低于对照组(P＜0。05)。两组患者的血脂指标TG、TC、LDL-C均较治疗前下降(P＜0。05)，且治疗组低于对照组(P＜0。05);两组患者的HDL-C水平均较治疗前上升(P＜0。05)，且治疗组高于对照组(P＜0。05)。两组患者的胰岛素抵抗指数均较治疗前降低(P＜0。05)，且治疗组低于对照组(P＜0。05)。治疗后两组患者的中医证候积分均较治疗前下降(P＜0。05)，且治疗组低于对照组(P＜0。05)。治疗组的临床总有效率高于对照组(P＜0。05)。结论:大柴胡汤联合针刺疗法治疗脉躁型2型糖尿病能提高临床疗效，有效改善患者的血糖、血脂水平以及胰岛素抵抗相关指标，值得推广。
Clinical observation on treating type 2 diabetes with the Dachaihu decoction plus acupuncture under the guidance of pulse-respiration technique of Changsang Jun pulse method
Objective:To study the clinical effect of the Dachaihu decoction(大柴胡汤)plus acupuncture on type 2 diabetes under the guidance of pulse-respiration technique of Changsang Jun pulse method,in order to provide new ideas for the clinical treatment of the disease.Methods:The pulse-respiration rate of type 2 diabetes patients in Liling Hospital of Traditional Chinese Medicine from January to December 2023 was detected by pulse-respiration technique of Changsang Jun pulse method,and a total of 60 type 2 diabetes patients with agitating pulse were screened out and divided into the treatment group and the control group by random number table method,with 30 patients in each group.The treatment group were treated with the Dachaihu decoction plus acupuncture,while the control group were treated by conventional Western medicine.After treatment,the blood glucose indexes(GLU,2 h PBG,HbA1 c),blood lipids indexes(TG,TC,LDL-C,HDL-C),insulin resistance,TCM symptom scores,and clinical efficacy were observed in the two groups.Results:After 8 weeks of treatment,the blood glucose related indicators(GLU,2 h PBG,HbA1 c)of patients in both groups were decreased compared with before treatment(P＜0.05),and in the treatment group was lower than the control group(P＜0.05).The levels of blood glucose indexes such as TG,TC,LDL-C of patients in both groups were decreased compared with before treatment(P＜0.05),and in the treatment group were lower than the control group(P＜0.05).The levels of HDL-C of patients in both groups were increased compared with before treatment(P＜0.05),and in the treatment group was higher than the control group(P＜0.05).The insulin resistance index of patients in both groups were decreased compared with before treatment(P＜0.05),and in the treatment group was lower than the control group(P＜0.05).The total clinical efficiency in the treatment group was higher than that in the control group(P＜0.05).Conclusion:The Dachaihu decoction plus acupuncture therapy on type 2 diabetes can effectively improve the clinical efficacy,effectively ameliorate blood glucose,blood lipid,and insulin resistance related indicators of patients,which is worth promoting and applying in the treatment of type 2 diabetes.
