目的:系统评价黄葵胶囊治疗肾病综合征的疗效与安全性。方法:通过计算机全面检索4个中文数据库中国知网、维普、万方、中国生物医学文献数据库，以及4个英文数据库PubMed、The Cochrane Library、Web of Science、Embase，收集自建库以来至2024年2月29日所有关于黄葵胶囊治疗肾病综合征的随机对照试验研究。分别由2位研究者独立进行文献筛选、数据提取，使用Cochrane风险偏倚评估工具进行文献质量评价，若出现分歧，则通过双方讨论解决或征求第三方意见协商达成共识，并使用STATA 15。0和RevMan5。4软件进行Meta分析。结果:最终纳入14项随机对照试验研究，共涉及879例患者，其中治疗组(黄葵胶囊联合常规西药治疗)439例、对照组(常规西药治疗)440例。Meta分析的结果显示:与常规西药治疗相对比，联合使用黄葵胶囊能显著降低肾病综合征患者的24小时尿蛋白定量[SMD=-1。34，95％CI(-1。88，-0。81)]，降低血清肌酐[SMD=-0。52，95％CI(-0。85，-0。19)]、尿素氮[SMD=-2。00，95％CI(-3。18，-0。82)]、三酰甘油[WMD=-0。28，95％CI(-0。48，-0。07)]、胆固醇[WMD=-1。45，95％CI(-1。88，-1。01)]、不良反应发生率[RR=0。63，95％CI(0。40，1。00)]，升高血清白蛋白[WMD=4。30，95％CI(2。85，5。76)]、总有效率[RR=1。22，95％CI(1。13，1。30)]。结论:在肾病综合征的治疗中，常规西药加用黄葵胶囊治疗可显著降低蛋白尿、改善肾功能、降低血脂、降低不良反应发生率、升高血清白蛋白、提高总有效率。然而，黄葵胶囊治疗肾病综合征的疗效仍需更多高质量、多中心、大样本的随机对照试验进一步证实。
The efficacy and safety of the Huangkui capsules in the treatment of nephrotic syndrome:a systematic review and Meta-analysis
Objective:To systematically evaluate the efficacy and safety of the Huangkui capsules(黄葵胶囊)in the treatment of nephrotic syndrome.Methods:Four Chinese databases of CNKI,VIP,Wanfang and CBM,and four English databases of PubMed,The Cochrane Library,Web of Science and Embase were searched to collect all RCTs on the treatment of nephrotic syndrome with Huangkui capsules from the establishment of each database to February 29,2024.Two review authors independently screened the literature,extracted data,and used the Cochrane to evaluate the quality of the literature.When there were differences of opinion,the two sides reached a consensus through discussion or consultation with third parties,and used STATA 15.0 and RevMan 5.4 software for Meta-analysis.Results:Fourteen RCTs involving a total of 879 patients were included.Among them,there were 439 cases in the treatment group(the Huangkui capsules combined with conventional Western medicine)and 440 cases in the control group(conventional Western medicine treatment).The results of Meta-analysis showed that,compared with the conventional Western medicine treatment,the combination of the Huangkui capsules significantly reduced the quantitative 24-hour urine protein[SMD=-1.34,95％CI(-1.88,-0.81)],reduced serum creatinine[SMD=-0.52,95％CI(-0.85,-0.19)],urea nitrogen[SMD=-2.00,95％CI(-3.18,-0.82)],triglycerides[WMD=-0.28,95％CI(-0.48,-0.07)],cholesterol[WMD=-1.45,95％CI(-1.88,-1.01)],and the incidence of adverse reactions[RR=0.63,95％CI(0.40,1.00)],elevated serum albumin[WMD=4.30,95％CI(2.85,5.76)],and the total efficiency[RR=1.22,95％CI(1.13,1.30)].Conclusion:In the treatment of nephrotic syndrome,the addition of the Huangkui capsules to conventional Western medicine treatment can significantly reduce proteinuria,improve renal function,reduce blood lipids,reduce the incidence of adverse reactions,increase serum albumin,and improve the total efficiency.However,the efficacy of the Huangkui capsules in the treatment of nephrotic syndrome still needs more RCTs of high-quality,multi-center,large-sample to further confirm.
