Treating chronic obstructive pulmonary disease plus sarcopenia based on the pectoral Qi theory of ZHANG Xichun
Chronic obstnictive pulmonary disease(COPD)is mainly characterized by persistent airflow limitation and the corresponding respiratory symptoms.It is a common airway disease in the Respiratory Department,and is also a complex multi-system disease,with a variety of internal and external pulmonary manifestations,which may further aggravate the risk factors of COPD and affect its prognosis.Sarcopenia is a common complication of COPD,with the decline of muscle mass and strength associated with aging as the main clinical manifestation.The disease limits body activity and endurance by affecting respiratory and non-respiratory muscle groups,and can seriously affect the prognosis of COPD.The pathogenesis of COPD plus sarcopenia is considered by most scholars to be related to systemic inflammatory response,oxidative stress,skeletal muscle metabolism disorder,glucocorticoid application,malnutrition,etc.,and the clinical therapeutic effect of existing therapies is not ideal.COPD plus sarcopenia belongs to the categories of lung swelling and flaccid paralysis categories in TCM.Based on the pectoral Qi(气)theory of TCM master ZHANG Xichun,and combined with the descriptions of generation and function of pectoral Qi in Yixue Zhongzhong Canxi Lu(《医学衷中参西录》),the author deeply explores new ideas of treating COPD plus sarcopenia in TCM,and believes that Qi deficiency and subsidence leads to systemic viscera gasilication function disorder,and endogenous phlegm and blood stasis is the fundamental etiology,with phlegm,blood stasis,and deficiency as the main pathogenesis.Qi deficiency and subsidence is root cause,and phlegm and blood stasis retardation is superficial symptom,and pectoral Qi deficiency and subsidence run through the whole course of the disease.From the aspect of the therapeutic principles of benefiting Qi and raising sunken Yang(阳),harmonizing Ying(营)and Wei(卫),regulating five Zang(脏)viscera simultaneously,facilitating Qi to dispel phlegm and blood stasis,the treatment of COPD in TCM is discussed in this paper,providing a comprehensive program with TCM characteristics and theoretical basis for the clinical diagnosis and treatment of the disease,and providing a valuable reference for improving the prognosis and quality of life of patients.
