目的:探究二仙壮骨汤与生骨胶囊口服结合人工股骨头置换术在老年股骨近端(髋部)骨质疏松骨折中的临床应用效果.方法:选择2020年3月-2023年2月在湖南省湘潭市中医医院就诊的老年骨质疏松性髋部骨折行人工股骨头置换手术的患者90例,按照随机数字表法分为试验组、对照组1、对照组2.试验组患者予院内自拟方二仙壮骨汤(经验方)及自制剂生骨胶囊口服,对照组1予阿仑膦酸钠片口服,对照组2不予特殊治疗,治疗时间为24周,对比三组患者用药前后中医证候积分、髋关节功能、骨密度等指标的差异.结果:试验组患者治疗后中医证候积分较治疗前显著降低,且试验组患者中医证候积分显著低于其他两组,差异有统计学意义(P＜0.05);对照组1与对照组2患者中医证候积分的差异无统计学意义(P＞0.05).试验组、对照组1患者治疗后髋关节功能各项评分、骨密度、碱性磷酸酶(Alkaline Phosphatase,ALP)、Ⅰ型前胶原氨基端前肽(Procollagen I N-Terminal Propeptide,PINP)、骨钙素(Bone Glaprotein,BGP)水平显著高于治疗前,且骨密度、ALP、PINP、BGP 水平显著高于对照组2,差异有统计学意义(P＜0.05),但试验组与对照组1的差异无统计学意义(P＞0.05).结论:二仙壮骨汤联合生骨胶囊口服可以有效改善骨质疏松型髋部骨折行人工股骨头置换术后患者的临床症状及髋关节功能,提高骨密度及骨转化指标水平.
A clinical study on treating senile proximal femoral osteoporotic fracture with the Erxian Zhuanggu decoction plus the Shenggu capsules and artificial femoral head replacement
Objective:To explore the clinical application effect of the Erxian Zhuanggu decoction(二仙壮骨汤)plus the Shenggu capsules(生骨胶囊)and artificial femoral head replacement on senile proximal femoral osteoporotic fractures in elderly patients.Methods:Ninety cases of senile proximal femoral osteoporotic fractures received artificial femoral head replacement surgery from March 2020 to February 2023 were selected and randomly divided into three groups according to the random number table method.Patients in the experimental group were given the self-prescribed formulas,the Erxian Zhuanggu decoction(empirical formula)and the Shenggu capsules orally,while the control group 1 was given oral administration of alendronate sodium tablets,the control group 2 received no special treatment.The treatment lasted for 24 weeks,and the differences in TCM syndrome scores,hip joint function,and bone mineral density before and after treatment were compared among the three groups.Results:The TCM syndrome scores of patients in the experimental group after treatment were significantly reduced compared with those before treatment,and the TCM syndrome scores of patients in the experimental group were significantly lower than those in the other two groups,with statistically significant differences(P＜0.05).There was no difference in the TCM syndrome score between the control group 1 and the control group 2(P＞0.05).After treatment,the hip joint function scores,bone mineral density,ALP,PINP,and BGP of patients in the experimental group and the control group 1 were significantly higher than those before treatment,and the bone mineral density,ALP,PINP,and BGP of patients in the experimental group and the control group 1 were significantly higher than those in the control group 2,with statistically significant differences(P＜0.05).However,there was no difference between the experimental group and the control group 1(P＞0.05).Conclusion:The Erxian Zhuanggu decoction plus the Shenggu capsule orally can effectively ameliorate the clinical symptoms and hip joint function of patients with osteoporotic fracture after artificial femoral head replacement surgery,and increase bone mineral density and bone turnover indicators.
