双黄连粉针剂与盐酸头孢吡肟联合用药的肾毒性研究

摘要
目的 观察双黄连粉针剂与注射用盐酸头孢吡肟联合用药对肾功能的影响.方法 将SD大鼠随机分为空白对照组,双黄连组,头孢吡肟高、中、低剂量组,双黄连合用头孢吡肟高、中、低剂量组.尾静脉注射给药7d,收集末次给药后24 h尿液,腹主动脉取血处死、摘除双肾检测丙二醛(MDA)、总超氧化物歧化酶(TSOD)以及尿β-D-N乙酰氨基葡萄糖苷酶(NAG)、尿蛋白等生化指标变化,并观察各组大鼠肾组织病理变化.结果 与空白对照组相比,合并用药各剂量组尿NAG、尿蛋白、肾组织MDA、组织T-SOD酶含量显著升高(P<0.01).合并用药组出现明显的肾小管蛋白管型及小血管壁增厚或增生.结论 双黄连粉针剂与注射用头孢吡肟联合应用会增加头孢吡肟的肾毒性.
Abstract
Objective To observe the renal injury after injection of Shuanghuanglian Injectable Powder combined with cefepime hydrochloride. Methods Sprague Dawley rats were randomly divided into eight groups: Shuanghuanglian Injectable Powder group, control group, high-, moderate- and low-dosage cefepime groups, and high-, moderate- and low-dosage combination groups of Shuanghuanglian Injectable Powder and cefepime. The drug was injected through caudal veins for one week. After the last injection, 24-hour urine was collected. Blood was sampled from the abdominal aorta of anesthetized rats and kidneys were taken out. The renal malondialdehyde( MDA), total superoxide dismutase(T-SOD), n-acetyl-β-d-glucosaminidase(NAG), and urine protein were measured. Moreover, the pathological changes of renal tissue were observed. Results Renal MDA and T-SOD contents, and urine protein and NAG contents were higher in the combination groups than those in the blank control group(P< 0.01). Large amount of obvious tubular protein cast and thickening or hyperplasia of small vessel walls was shown in the combination groups. Conclusion The combination of Shuanghuanglian Injectable Powder with cefepime hydrochloride injection will increase renal toxicity.
