Path analysis of the influence of disease perception and hope level on fear of disease progression in young and middle-aged patients with type 2 diabetes mellitus
Objective To investigate the current situation of fear disease progression in young and middle-aged patients with type 2 diabetes mellitus(T2 DM)and analyze its influencing factors,exploring the influence and mechanism of disease perception and hope level on fear disease progression.Methods A total of 408 young and middle-aged patients with T2 DM from three grade-A tertiary hospitals in Shijiazhuang were selected by convenience sampling method form June to October 2022.The subjects were investigated with the brief illness perception questionnaire(BIPQ),fear of progression questionnaire-short form(FoP-Q-SF)and Herth hope index(HHI).Results The score of fear disease progression,illness perception and hope level of the young and middle-aged patients with T2 DM were(31.29±5.17),(39.97±6.02)and(34.87±4.29)points,respectively.The fear of disease progression was positively correlated with illness perception(r=0.454,P<0.01)and negatively correlated with hope level(r=-0.526,P<0.01).The results of multiple regression analysis showed that gender,the frequency of blood glucose monitoring before hospitalization,last glycated hemoglobin,the history of hypoglycemia in the past six months,disease perception and hope level entered the regression equation(F=26.219,P<0.01),which could explain 38.30%of the fear of disease progression in young and middle-aged T2 DM patients.The path analysis revealed that hope level played a partial mediating effect between disease perception and fear of disease progression,accounting for 47.65%of the total effect.Conclusions The fear of disease progression in young and middle-aged patients with T2 DM is at an above medium level.Disease perception can indirectly affect the fear of disease progression in young and middle-aged patients with T2 DM through hope level.Clinical medical staff can develop targeted intervention measures and health guidance strategies to alleviate their fear of disease progression.
