目的 对核工业总医院钇-90(90Y)树脂微球治疗项目进行职业病危害放射防护控制效果评价,为项目的竣工验收和正式运行提供技术依据,为其他医疗机构建设该类项目提供参考.方法 2023年3-4月采用现场调查法、检查表分析法、综合分析法对该项目进行辐射源项分析、工作场所分区与分级、放射防护检测、表面污染检测及放射防护评价.结果 本项目投入运行后核医学科场所总日等效最大操作量约为1.01x108 Bq;场所周围环境辐射剂量率及β表面污染水平较高的为90Y药物分装过程,通风橱表面5 cm处X-γ剂量当量率最大值为8.79 μSv/h,β表面污染最大值为0.22 Bq/cm2,满足《核医学放射防护要求》.结论 本项目采取的各项措施均符合国家标准要求,放射防护控制效果合格,具备放射防护设施竣工验收的条件.使用放射性核素90Y、99Tcm进行治疗属于放射性危害程度与诊疗风险危害严重类的建设项目,医院应重视该项目的放射防护,切实保障放射工作人员及公众的辐射安全和健康.
Evaluation of radiation protection for 90Y resin microspheres treatment project
Objective To evaluate the effectiveness of radiation protection and control for the occupational hazards of the 90Y resin microsphere treatment project in the China Institute of Atomic Energy Hospital,providing technical basis for the completion acceptance and officially operation of the project,and reference for other medical institutions building similar projects.Methods From March to April 2023,the on-site investigation method,checklist analysis method and comprehensive analysis method were used to analyze radiation source items,workplace zoning and grading,radiation protection detection,surface contamination detection and radiation protection evaluation of the project.Results After the project was put into operation,the total daily equivalent maximum operation volume of the nuclear medicine department site was about 1.01×108 Bq.The radiation dose rate and β surface contamination level of the surrounding environment of the site were higher for 90Y drug dispensing process,the maximum X-γ dose equivalent rate at 5 cm on the surface of the fume hood was 8.79 μSv/h,and the maximum β surface pollution was 0.22 Bq/cm2,which met the requirements of nuclear medicine radiation protection.Conclusions The measures adopted in this project all meet the requirements of national standards,the radiation protection control effect is qualifiied,and the conditions for the completion and acceptance of radiation protection facilities are met.The project uses radioactive nuclides 90Y、99Tcm for treatment,which belongs to the construction project category of radioactive hazard and serious diagnostic and treatment risk.The hospital should attach importance to the radiation protection of this project and ensure the radiation safety and health of radiation workers and the public.
