天麻钩藤饮对肾血管性高血压大鼠心肌纤维化及AngⅡ和ALDO的影响

目的:探讨天麻钩藤饮对肾血管性高血压大鼠心肌纤维化及醛固酮和血管紧张素Ⅱ的影响。方法:二肾一夹肾血管性高血压大鼠造模后第5周开始灌胃给药,连续治疗8周,观察各组大鼠血压、左室重量(LVW)、左室重量指数(LVI=LVw/BW)和心肌组织胶原浓度(Coll),观察天麻钩藤饮降压和抑制心肌纤维化的作用。同时观察各组大鼠血浆和心肌组织中AngⅡ和ALDO水平。结果:造模4周后,模型组大鼠血压明显上升,左肾重量(LK)和LK/RK值比假手术组明显降低,而右肾重量(RK)则显著增高,说明动物模型复制成功。8周干预治疗后,卡托普利组和天麻钩藤饮组大鼠血压均下降至达到或接近正常血压水平。模型组大鼠的LVW、LVI和Coll均明显高于假手术组,说明造模后12周出现了明显的左室肥厚和心肌纤维化现象,卡托普利组和天麻钩藤饮组大鼠的上述指标均有显著降低。造模12周后,模型组大鼠血浆ALDO和AngⅡ水平明显高于假手术组,卡托普利显著降低了血浆AngⅡ水平,而对血浆ALDO水平则没有影响。天麻钩藤饮则同时降低了ALDO和AngⅡ水平。模型组大鼠心肌ALDO和AngⅡ水平较假手术组明显上升,卡托普利只对AngⅡ具有显著的降低作用,天麻钩藤饮则对ALDO和AngⅡ均有明显的抑制作用。结论:天麻钩藤饮能够明显降低肾性高血压大鼠的血压水平,并且能够缓解和逆转左室肥厚和心肌纤维化。其作用可能同降低血浆和心肌局部ALDO、AngⅡ水平有关。
The effect of Tianma Gouteng Decoction(天麻钩藤饮).on myocardial fibrosis and change of Ang Ⅱ and ALDO in renovascular hypertensive rats
To study the effect of Tianma Gouteng Decoction on myocardial fibrosis and change of Ang II and ALDO in renovascular hypertensive rats. Methods:48 male SD rats were randomly grouped to the sham-operation group, the model group, the captopril group and the Tianma Gouteng Decoction group, 12 every group. From beginning of the fifth week, the rats were treated with drugs and persisted in 8 weeks. Then observed the BP, LVW, LVI and Coll in every group to confirmed the effect of decreasing blood pressure and reversing myocardial fibrosis. At same time, we observed the change of Ang II and ALDO in circulation and cardiac muscle in every group to probe into the mechanism of treatment. Results: After 4 weeks, the blood pressure of the rats of the model group ascended obviously (P<0.001) , left kidney weight(LK) ,LK/RKdescended(P<0.001) and right kidney weight(RK) increased obviously(P <0.001) , these all confirmed that the operation succeed. After 8 weeks of treatment, the blood pressure level of the captopril and the Tianma Gouteng Decoction group decreased to obtain or be close to normal blood pressure( P < 0. 01). The effect of descending blood pressure of captopril is better (P < 0.05 ). In 12 weeks, the LV W, LVI and Coll of the model group were all higher than the sham-operation group (allP<0.01). It showed that left ventricular hypertrophy and myocardial fibrosis appeared. Those indexes all decreased in the captopril and the Tianma Gouteng Decoction group, but there were not distinct difference between the two groups(P > 0.05). After 12 weeks, the level of ALDO and Ang II in plasma of the model group were higher than the sham-operation group obviously (P < 0.001, P < 0.01). Captopril can decreased the level of the Ang II in plasma distinctly(P<0.05) , but ALDO was not affected(P >0.05). Tianma Gouteng Decoction can decreased the level of ALDO and Ang II in plasma at same time( P < 0.01, P < 0. 05 ). The level of ALDO and Ang II in cardiac muscle of the model group were all higher than the sham-operation group obviously( P < 0.001, P < 0.05 ) , captopril only can decreased the level of Ang II (P < 0.05) and that Tianma Gouteng Decoction can affected both ALDO and Ang II(P<0.01,P<0.05). Conclusion: Through the research, we confirmed that Tianma Gouteng Decoction can decrease the blood pressure of the renovascular hypertensive rat and reverse left ventricular hypertrophy and myocardial fibrosis. The effects maybe relate with the decreasing of the level of the ALDO and Ang II in circulation and cardiac muscle.
