补肺汤对COPD大鼠模型中性粒细胞胞外诱捕网及维生素D的影响

目的:观察补肺汤对慢性阻塞性肺疾病(COPD)模型大鼠中性粒细胞胞外诱捕网(NETs)相关组分以及维生素D水平的影响,探究补肺汤治疗COPD的作用机制。方法:70只SD大鼠随机分为:正常对照组、模型对照组、乙酰半胱氨酸0。1 g/kg组、维生素D 3×108 g/kg组、补肺汤4。5、9、18 g/kg组,每组10只,除正常对照组外,其余各组采用脂多糖(LPS)联合烟熏法建立COPD大鼠模型。采用HE染色法对肺组织进行病理结构观察;ELISA法检测血清肿瘤坏死因子-α(TNF-α)、髓过氧化物酶(MPO)、25羟维生素D3[25(OH)D3]以及NETs含量;Western blot法和免疫组化(IHC)法检测肺组织中中性粒细胞弹性蛋白酶(NE)、瓜氨酸组蛋白H3(CitH3)、MPO、肽酰基精氨酸脱亚氨酶4(PAD4)和维生素D受体(VDR)蛋白阳性表达。结果:与正常对照组相比,模型对照组大鼠症状、体征表现较为严重,肺组织病理结构紊乱,肺泡肿大且部分肺泡膈断裂导致肺泡融合,气管周围有炎性细胞浸润并伴有增生结缔组织,血清中的TNF-α、MPO、NETs含量显著升高,25(OH)D3含量显著降低(P<0。01);肺组织中NE、CitH3、MPO、PAD4蛋白表达显著升高,VDR蛋白表达显著下降(P<0。01);与模型对照组相比,乙酰半胱氨酸0。1 g/kg组、维生素D 3×10-8g/kg组以及补肺汤9、18 g/kg组大鼠症状、体征明显好转,肺组织病理损伤可见不同程度改善,血清TNF-α、MPO、NETs含量明显降低,25(OH)D3含量明显升高(P<0。05或P<0。01);肺组织中NE、CitH3、MPO、PAD4蛋白表达明显降低,VDR蛋白表达明显升高(P<0。05或P<0。01)。结论:补肺汤可有效减轻COPD炎症反应并改善肺损伤,其作用机制可能与调控维生素D水平从而抑制促炎因子的释放以及NETs相关蛋白表达有关。
Effects of Bufei(补肺)Decoction on Extracellular Neutrophil Traps and Vitamin D in COPD Rats
Objective:To observe the effects of Bufei(补肺)Decoction on the components related to extracellular neutrophil traps(NETs)and the level of vitamin D in the rat model of chronic obstructive pulmonary disease(COPD)and explore the mechanism of Bufei Decoction in trea-ting COPD.Methods:Seventy SD rats were randomized into blank control,model,acetylcysteine(0.1 g/kg),vitamin D(3×10-8 g/kg),Bufei Decoction(4.5,9,and 18 g/kg)groups,with 10 rats in each group.The rats in other groups except the blank control group were mod-eled for COPD by lipopolysaccharide(LPS)combined with smoking.Hematoxylin-eosin staining was conducted to observe the pathological changes in the lung tissue.The levels of tumor necrosis factor-α(TNF-α),myeloperoxidase(MPO),25-hydroxyvitamin D3(25(OH)D3),and NETs in the serum were measured by enzyme-linked immunosorbent assay.The expression of neutrophil elastase(NE),citrullinated his-tone H3(CitH3),MPO,peptidyl arginine deiminase 4(PAD4),and vitamin D receptor(VDR)in the lung tissue was determined by West-em blotting and immunohistochemistry(IHC).Results:Compared with the blank control group,the model group showed serious symptoms and signs,disarranged structure of the lung tissue,alveolar swelling and alveolar fusion due to partial alveolar diaphragm rupture,infiltration of inflammatory cells around trachea with hyperplasia of the connective tissue.Furthermore,the modeling raised the levels of TNF-α,MPO,and NETs and lowered the level of 25(OH)D3 in the serum(P<0.01),up-regulated the protein levels and average optical density of NE,CitH3,MPO,and PAD4 and down-regulated the protein level and average optical density of VDR in the lung tissue(P<0.01).Compared with the model control group,acetylcysteine(0.1 g/kg),vitamin D(3×10-8 g/kg),and Bufei Decoction(9 g/kg and 18 g/kg)ameliorated the symp-toms and signs,mitigated the pathological damage in the lung tissue,lowered the levels of TNF-α,MPO and NETs and elevated the level of 25(OH)D3 in the serum(P<0.01 or P<0.05).At the same time,the drugs down-regulated the protein levels and average optical density of NE,CitH3,MPO,and PAD4 and up-regulated the protein level and average optical density of VDR in the lung tissue(P<0.01 or P<0.05).Conclusion:Bufei Decoction can reduce the inflammation and lung damage in the rat model of COPD by regulating the level of vitamin D to inhibit the release of pro-inflammatory cytokines and the expression of NET-related proteins.
