从低度炎症角度论述腹泻型肠易激综合征相关性的中西医研究进展

摘要
近年来研究发现腹泻型肠易激综合征(Irritable bowel syndrome,IBS)是肠易激综合征(IBS with predominant diarrhea,IBS-D)中最为常见的类型之一,其发病机制复杂尚未完全明确.对症治疗时短期药物疗效尚可,但长期效果不佳,停药后易反复.近年来,中医药被证明在治疗腹泻型肠易激综合征中临床疗效显著,中医药多靶点、多途径、多通路的治疗机制可以通过降低肠道低度炎症,缓解腹泻型肠易激综合征的临床症状.结合目前最新专家共识认为低度炎症是影响腹泻型肠易激综合征患者发病机制的重要因素,本文通过检索近年来国内外相关文献,尝试从不同角度归纳总结低度炎症对于IBS-D的影响,发现其潜在的干预机制和治疗价值,为中西医结合治疗IBS-D提供理论参考和依据.
Abstract
Recent studies have found that diarrheal irritable bowel syndrome is one of the most common types of irritable bowel syndrome.Its patho-genesis is complex and multidimensional and is not fully understood.Short-term symptomatic treatment with the drug has certain efficacy but poor long-term effect,and the disease is easy to repeat after withdrawal.In recent years,traditional Chinese medicine has been proven to have significant clinical efficacy in the treatment of diarrheal irritable bowel syndrome.The multi-target,multi-approach,and multi-pathway thera-peutic mechanism of traditional Chinese medicine can alleviate the clinical symptoms of diarrheal irritable bowel syndrome by reducing low-grade intestinal inflammation.According to the latest expert consensus,low-grade inflammation is an important factor affecting the pathogene-sis of diarrheal irritable bowel syndrome.By searching the relevant literature in China and abroad in recent years,this paper attempted to ex-plore the effects of low-grade inflammation on diarrheal irritable bowel syndrome from different perspectives and find its potential intervention mechanism and therapeutic value,so as to provide a theoretical reference and basis for the treatment of diarrheal irritable bowel syndrome with both Chinese and Western medicine.
