中药调控PI3K/AKT信号通路治疗银屑病作用机制研究进展

银屑病是一种免疫介导的慢性炎症性疾病,治疗难度大且易复发,给患者造成巨大困扰。磷脂酰肌醇3-激酶(PI3K)/蛋白激酶B(AKT)信号通路作为维持细胞稳态的核心枢纽,与银屑病发病机制关系密切,可能涉及调控角质形成细胞异常增殖和分化;下调血管内皮生长因子(VEGF)表达,抑制微血管生成;激活下游信号哺乳动物雷帕霉素靶蛋白(mTOR),诱导细胞自噬反应等生物学过程。目前,西医多采用生物制剂、小分子靶向药物对银屑病进行治疗,但疗效有限,且存在依赖性和不良反应。中药在银屑病治疗方面已经取得了良好的效果,不仅安全性高,而且副作用少,受到患者广泛关注。当前中药单体有效成分及中药复方对银屑病的治疗主要以实验和临床研究为主,对其致病机制缺乏系统性的阐述和剖析,本文以PI3K/AKT信号途径为切入点,对现有中药单体及中药复方调控该通路干预发病机制治疗银屑病的最新国内外研究进行总结并归纳,以期为中药治疗银屑病及临床新药研发提供更多理论依据。
Chinese Medicines Regulate PI3K/AKT Signaling Pathway to Treat Psoriasis:A Review
Psoriasis,an immune-mediated chronic inflammatory disease,is difficult to be cured and prone to relapse,causing trouble to the patients.Phosphatidylinositol 3-kinase (PI3K)/protein kinase B (AKT) signaling pathway,as the hub to maintain cell homeostasis,is closely related to the pathogenesis of psoriasis,and it may be involved in the regulation of abnormal proliferation and differentiation of keratinocytes.This pathway can down-regulate the expression of vascular endothelial growth factor (VEGF) to inhibit angiogenesis,and it can activate down-stream mammalian target of rapamycin (mTOR) to induce autophagy.At present,the Western medical therapies for psoriasis mainly include biological agents and small-molecule targeted drugs,which showcase limited therapeutic effects and may induce drug dependence and adverse reactions.Chinese medicines have achieved good results in the treatment of psoriasis because of the high safety and mild side effects,gaining increasing attention from patients.At present,experimental and clinical studies have been carried out regarding the treatment of psoriasis with Chinese medicines active components and compound prescriptions,while the pathogenesis of this disease remains to be systematically ana-lyzed.This paper reviews the studies about the treatment of psoriasis with Chinese medicine active components and compound prescriptions via regulating the PI3K/AKT signaling pathway,aiming to provide a theoretical basis for the treatment of psoriasis with Chinese medicines and the development of new drugs.
