腰痛活血胶囊对强直性脊柱炎Th1/Th2型趋化因子的影响

摘要
目的:观察并研究腰痛活血胶囊对强直性脊柱炎患者疾病活动和趋化因子的影响.方法:选取2021年1月-2023年1月我科住院的明确诊断为强直性脊柱炎患者60例,进行随机对照实验,对比两组的临床疗效、疼痛指数评分(VAS评分)、疾病活动量表评分(BASDAI、BASFI、BASMI、MASES)、情绪生活质量评分(HAMD、ASQoL),测定血清炎症指标(ESR、hs-CRP)、趋化因子含量(TARC、IP-10、CXCR3)和细胞因子IL-10含量.结果:两组患者治疗后症状均有所改善,运用中成药腰痛活血胶囊治疗的AS患者临床疗效优于对照组(P＜0.05),疾病活动评分较对照组下降明显(P＜0.05),ESR和hs-CRP含量明显降低(P＜0.05),HAMD评分和ASQoL评分明显下降(P＜0.05),趋化因子TARC、IP-10及受体CXCR3含量明显下降(P＜0.05),细胞因子IL-10明显升高(P＜0.05).Spearman 相关分析结果显示,IP-10 与 VAS 呈正相关(r=0.89,P＜0.05),IL-10 水平与 BASDAI 呈负相关(r=-0.40,P＜0.05).结论:腰痛活血胶囊治疗强直性脊柱炎临床应用有效,可有效降低疾病活动,改善患者情绪,提高生活质量,调节免疫.
Abstract
Objective:To study the effects of Yaotong Huoxue(腰痛活血)Capsule on disease activity and chemokines in patients with ankylosing spondylitis(AS).Methods:Sixty patients with definite diagnosis of ankylosing spondylitis who were hospitalized in our department from Jan-uary 2021 to January 2023 were selected for a randomized controlled trial.The clinical efficacy,pain visual analog scale(VAS),disease ac-tivity scores(bath ankylosing spondylitis disease activity index(BASDAI),bath ankylosing spondylitis functional index(BASFI),bath anky-losing spondylitis metrology index(BASMI),Maastricht ankylosing spondylitis enthesitis score(MASES)),emotional quality of life scores(Hamilton Rating Scale for Depression(HAMD),ankylosing spondylitis quality of life(ASQoL)),serum inflammatory indexes(erythrocyte sedimentation rate(ESR),high-sensitivity C-reactive protein(hs-CRP)),chemokine levels(thymus and activation-regulated chemokine(TARC),Interferon-γ-inducible protein 10(IP-10),CXC chemokine receptor 3(CXCR3)),and interleukin 10(IL-10)level were com-pared between the two groups.Results:After the treatment,symptoms were improved in two groups,and the clinical efficacy of AS patients treated with the Chinese patent medicine Yaotong Huoxue Capsule was better than that of the control group(P＜0.05).Compared with control group,the disease activity scores were decreased significantly(P＜0.01),and ESR and hs-CRP levels were increased significantly(P＜0.01),with significantly decreased HAMD and ASQoL scores(P＜0.01),and TARC,IP-10 and CXCR3 levels(P＜0.01),and significantly increased IL-10 level(P＜0.01)in the Yaotong Huoxue Capsule treatment group.Spearman's correlation analysis showed that serum IP-10 and CXCR3 levels were positively correlated with BASDAI after treatment,and IP-10 level was positively correlated with VAS(r=0.89,P＜0.05).While IL-10 level was negatively correlated with BASDAI(r=-0.40,P＜0.05).Conclusion:Yaotong Huoxue Capsule is effective in the clinical treatment of AS,which can effectively reduce disease activity,improve patients'mood and quality of life,and regulate the function of the immune system.
