心力衰竭(Heart failure,HF)是各种心血管疾病的终末期阶段,病程呈渐进性加重,早期诊治HF可以提高患者生存质量并减少医疗支出.为实现HF早期诊治,现代医学将HF分为不同时期,即A期、B期、C期和D期,并制定了相应临床指南.射血分数保留心力衰竭(Heart failure with preserved ejection fraction,HFpEF)是心力衰竭的一种类型,HFpEF早期防治缺乏充分证据支持.中医药在HFpEF的早期防治中具有潜力,但由于缺乏明确分期标准,临床研究发展受到限制.中国研究型医院学会中西医结合心血管病专家委员会、国家中医心血管病临床医学研究中心和中国中医药研究促进会青年医师分会发布了《射血分数保留心衰中西医病证结合的分期诊断专家共识》(以下简称《共识》),该共识基于文献调研和专家意见,以病为依托,注重证的发展规律和临床特征,提供了 HFpEF分期诊断的指导框架,旨在探讨《共识》的制定动机和突出特点,以帮助各位同仁能更好地理解和运用《共识》,为实际临床工作提供指导和启示.
Interpretation of Expert Consensus on Staging Diagnosis of Heart Failure with Preserved Ejection Fraction Integrating Traditional Chinese and Western Medicine
Heart failure(HF)represents the final stage of various cardiovascular diseases,characterized by progressive dete-rioration.Early diagnosis and treatment of HF can enhance patients'quality of life and reduce healthcare costs.To achieve early diagnosis and treatment,modern medicine classifies HF into different stages:Stage A,Stage B,Stage C and Stage D,each with the corresponding clinical guidelines.Heart failure with preserved ejection fraction(HFpEF)is a specific type of HF,but currently,there is insufficient evidence regarding its early prevention and treatment.Traditional Chinese medicine shows potential in this re-gard,but the absence of clear staging criteria hampers the advancement of clinical research in this field.To address these issues,the Cardiovascular Disease Expert Committee of the Chinese Research Hospital Association for Integrative Medicine,the National Clinical Research Center for Traditional Chinese Medicine in Cardiovascular Disease,and the Youth Physician Branch of the Chi-nese Association for the Promotion of Traditional Chinese Medicine have collaboratively released The Expert Consensus on the Staging Diagnosis of Heart Failure with preserved ejection fraction Integrating Chinese and Western Medicine(hereinafter referred to as the Consensus).This Consensus,based on literature research and expert opinions,focuses on the development patterns and clinical characteristics of syndromes,providing a framework for the staging diagnosis of HFpEF.The purpose of this article is to explore the motivation and prominent features of the Consensus,aiming to help colleagues gain a better understanding and applica-tion of it while providing guidance and inspiration for clinical work.
