温阳健脾化痰汤辅助治疗支气管扩张疗效及对HNP1-3、IL-17变化的研究

摘要
目的 研究温阳健脾化痰汤辅助治疗支气管扩张的疗效及对患者α-防御素1-3(HNP1-3)、白细胞介素-17(IL-17)的变化影响.方法 选取诸暨市人民医院2020年10月—2022年4月收治的支气管扩张缓解期患者86例,采用随机数字法分为两组.对照组给予体位引流、盐酸氨溴索片等常规治疗,汤剂组加用温阳健脾化痰汤治疗,连续治疗4周.分析两组治疗4周后的疗效,检测两组肺功能、相关血清因子及圣乔治呼吸问卷(SGOR)评分、6 min步行距离(6MWT)的差异.结果 汤剂组治疗4周后中医证候总有效率(90.70％,39/43)高于对照组(74.42％,32/43),差异有统计学意义(P＜0.05).两组治疗前肺功能指标比较,差异无统计学意义(P＞0.05).汤剂组治疗4周后第1秒用力呼气量(FEV1)[(2.86±0.42)L]、用力肺活量(FVC)[(2.96±0.33)L]、FEV1/FVC(60.47％±6.89％)水平高于对照组(P＜0.05).两组治疗前HNP1-3、白细胞介素-2(IL-2)、白细胞介素-8(IL-8)、IL-17水平比较,差异无统计学意义(P＞0.05).汤剂组治疗4周后除IL-2水平高于对照组外,其他因子水平低于对照组(P＜0.05).两组治疗前SGOR评分、6MWT比较,差异无统计学意义(P＞0.05).汤剂组治疗4周后6MWT[(394.11±70.14)m]长于对照组,SQRQ评分[(12.41±3.65)分]低于对照组(P＜0.05).结论 温阳健脾化痰汤辅助治疗支气管扩张可改善肺功能,减轻炎症反应,提高生活质量及运动耐力.
Abstract
Objective To study the effect of Wenyang Jianpi Huatan Decoction(温阳健脾化痰汤)on bronchodilatation and the changes of alpha-defensin 1-3(HNP1-3)and interleukin-17(IL-17)in patients.Methods Eighty-six patients in re-mission stage of bronchiectasis in Zhuji People's Hospital from October 2020 to April 2022 were selected and randomly divided in-to two groups.The control group was treated with body position drainage,ambroxol hydrochloride tablets and other conventional treatments,while the decoction group was treated with Wenyang Jianpi Huatan Decoction for 4 consecutive weeks.The therapeutic effects of the two groups after 4 weeks of treatment were analyzed,and the differences in lung function,related serum factors,St.George's Respiratory Questionnaire(SGOR)scores and 6-minute walking distance(6MWT)between the two groups were detec-ted.Results After 4 weeks of treatment,the total effective rate of TCM symptoms in the decoction group was 90.70％(39/43),higher than that in the control group[74.42％(32/43)],with a difference(P＜0.05).There was no difference in pulmonary function between the two groups before treatment(P＞0.05).The levels of forced expiratory volume(FEV1)[(2.86±0.42)L],forced vital capacity(FVC)[(2.96±0.33)L]and FEV1/FVC[(60.47±6.89)％]in the decoction group at the first second after 4 weeks of treatment were higher than those in the control group(P＜0.05).There was no difference in the levels of HNP1-3,interleukin-2(IL-2),interleukin-8(IL-8)or IL-17 between the two groups before treatment(P＞0.05).After 4 weeks of treatment,the levels of other factors in the decoction group were lower than those in the control group(P＜0.05),ex-cept that the level of IL-2 was higher than that in the control group.There was no difference in SGOR score and 6MWT between the two groups before treatment(P＞0.05).After 4 weeks of treatment,the 6MWT[(394.11±70.14)m]in the decoction group was longer than that in the control group,and the SQRQ score[(12.41±3.65)point]was lower than that in the control group(P＜0.05).Conclusion Wenyang Jianpi Huatan Decoction can improve lung function,reduce inflammation and improve life quality and exercise endurance.
