动脉粥样硬化(atherosclerosis，AS)是脂质沉积于血管壁导致的慢性炎症性疾病，是冠心病、高血压病、心力衰竭等心脑血管疾病的病理基础，动脉粥样硬化斑块的稳定或破裂是心血管事件发生与否的重要影响因素。细胞焦亡是一种促炎性质的程序性细胞死亡，过度的细胞焦亡可引起细胞裂解死亡并发级联炎症反应。越来越多的研究表明细胞焦亡在AS的病理进程中扮演重要角色，NOD样受体蛋白3(nod-like receptor protein 3，NLRP3)、半胱氨酸天冬氨酸蛋白酶-1(cysteinyl aspartate specific proteinase-1，Caspase-1)、细胞焦亡执行蛋白(gasdermin D，GSDMD)等细胞焦亡的关键蛋白参与AS血管内皮细胞损伤、内皮功能障碍、平滑肌细胞表型转换、巨噬细胞死亡、泡沫细胞形成等过程，已成为AS新的治疗靶点。中医药是历史悠久的医药学体系，随着研究的深入，中药复方及中药单体活性成分通过调控细胞焦亡干预AS的作用逐步被阐明，且具有多途径，多靶点，不良反应小等独特优势。基于上述研究，阐述细胞焦亡的概念及发生机制，归纳细胞焦亡在AS病程中的作用，并总结中药复方和单味中药有效成分基于细胞焦亡干预AS的研究进展，以期为AS的防治及中药新药的开发提供新的思路。
Research Progress on Role of Pyroptosis in Atherosclerosis and Regulation of Traditional Chinese Medicine
Atherosclerosis is a chronic inflammatory disease caused by lipid deposition in the vascular wall,which is the pathological basis of coronary heart disease,hypertension,heart failure and other cardiovascular and cerebrovascular diseases.The stability or rupture of atherosclerotic plaques is an important factor affecting the occurrence of cardiovascular events.Pyroptosis is a kind of programmed cell death with pro-inflammatory properties.The excessive pyroptosis can lead to cell lysis and death and a cascade of inflammatory reactions.More and more studies have shown that pyroptosis plays an important role in the pathological process of atherosclerosis.Nod-like receptor protein 3(NLRP3),cysteinyl aspartate protease-1(Caspase-1),gasdermin D(GSDMD)and other key proteins involved in atherosclerosis vascular endothelial cell injury,endothelial dysfunction,smooth muscle cell phenotypic transformation,macrophage death and foam cell formation and have become new therapeutic targets for AS.Traditional Chinese medicine(TCM)is a medical system with a long history,with the deepening of research,the role of TCM compounds and active components of TCM monomers in the intervention of atherosclerosis through the regulation of pyroptosis has been gradually clarified,and it has the unique advantages of multi-pathway,multi-target and small adverse reactions.Based on the above studies,this paper described the concept and mechanism of pyroptosis,summarized the role of pyroptosis in the course of atherosclerosis,and summarized the research progress of TCM compound and single Chinese medicine active ingredients on the intervention of atherosclerosis based on pyroptosis,in order to provide new ideas for the prevention and treatment of atherosclerosis and the development of new Chinese medicine drugs.
