炙甘草汤防治心血管疾病的实验与机制概述

心血管疾病是全球范围内的主要公共卫生问题,其发病率和死亡率居高不下。炙甘草汤是仲景治疗心悸的名方,经历代医家的实践拓展,现已广泛运用于各类心血管疾病的预防与治疗。多个动物和细胞实验研究的大量证据表明,炙甘草汤可从抗动脉粥样硬化、抗心肌纤维化、抗心律失常3个主要方面防治心血管疾病:①抑制炎症反应、减轻氧化应激、保护血管内皮,防治冠状动脉粥样硬化性心脏病;②通过降低心肌组织PC Ⅰ及PC Ⅲ浓度、调控NF-κB信号通路等途径抑制心肌纤维化;③作用于钠电流(INa)、L型钙电流(ICaL)、快速延迟整流钾电流(Ikr)、瞬时外向钾电流(Ito)等多个离子通道,延长了心肌细胞动作电位时程、降低了动作电位振幅;并可通过调节PI3K/Akt/mTOR信号通路,减轻缺血再灌注后心肌损伤的程度;因此有效减少了心律失常发生的次数、改善了发作时的严重程度。
Experiment and Mechanism of Zhigancao Decoction(炙甘草汤)in Prevention and Treatment of Cardiovascular Diseases
Cardiovascular disease is a major public health problem worldwide,and its incidence rate and mortality remain high.Zhigancao Decoction(炙甘草汤)is a famous prescription for treating palpitations by Zhongjing,and has been widely used in the prevention and treatment of various cardiovascular diseases through the practice and expansion of modern physicians.A large amount of evidence from multiple animal and cellular experimental studies shows that Zhigancao Decoction can prevent and treat cardiovascular diseases from three main aspects:anti atherosclerosis,anti myocardial fibrosis and anti arrhythmia.①It can inhibit inflammatory reaction,reduce oxidative stress,protect vascular endothelium and prevent and treat ischemic cardiomyopa-thy.②By reducing the concentrations of procollagen type Ⅰ(PC Ⅰ)and procollagen type Ⅲ(PC Ⅲ)in myocardial tissue,regu-lating nuclear factor(NF)-κB signal pathway and other pathways,it can inhibit myocardial fibrosis.③It can act on multiple ion channels such as sodium current(INa),L-type calcium current(ICaL),fast delayed rectifier potassium current(Ikr)and instanta-neous outward potassium current(Ito).It prolongs the action potential duration of myocardial cells and reduces the action poten-tial amplitude.It can alleviate the degree of myocardial injury after ischemia reperfusion by regulating the PI3K/Akt/mTOR sig-nal pathway.Therefore,it effectively reduces the occurrence of arrhythmia and improves the severity of the attack.
