目的 分析脑肠同调论针刺治疗急性脑梗死的血清代谢组学的影响。方法 纳入2018年3月—2022年12月医院收治的急性脑梗死患者126例作为研究对象，随机数字表法分为两组，对照组63例采用常规疗法，观察组63例在此基础上采用脑肠同调论针刺治疗，对比两组治疗效果、神经功能，炎症因子水平以及对血清代谢组学的影响。结果 观察组总有效率为96。83％(61/63)，远高于对照组的80。96％(57/63)，神经功能缺损评分(NIHSS)评分低于对照组(P＜0。05);治疗后观察组白介素-17(Interleukin-17，IL-17)、超敏 C 反应蛋白(hypersensitive C-reactive protein，hs-CRP)、血浆氧化三甲胺(trimethylamine oxide，TMAO)水平均低于对照组(P＜0。05);治疗后观察组苯丙氨酸、溶血卵磷脂、焦谷氨酸水平低于对照组(P＜0。05)。结论 脑肠同调论针刺治疗急性脑梗死效果明确，可减轻患者炎症反应，调节其肠道菌群代谢产物水平及氨基酸、磷脂代谢障碍等，使紊乱的代谢趋于正常，以达促进神经功能恢复，改善患者预后的目的，有较高应用价值。
Metabolomics Study on Acupuncture Treatment of Acute Cerebral Infarction Based on Theory of Brain Gut Homology
Objective To analyze the effect of cerebral and intestinal homology acupuncture on serum metabolomics of acute cerebral infarction.Methods A total of 126 patients with acute cerebral infarction admitted to the hospital from March 2018 to De-cember 2022 were included as research objects.They were divided into two groups by random number table method.Sixty-three patients in the control group received conventional therapy.The other sixty-three in the observation group were treated with cer-ebral and intestinal homology acupuncture,and the therapeutic effect,neurological function,inflammatory factor levels and the in-fluence on serum metabolomics were compared between the two groups.Results The total effective rate of the observation group was 96.83％(61/63),much higher than that of the control group 80.96％(57/63),and the National Institute of Health stroke scale(NIHSS)score was lower than that of the control group(P＜0.05).After treatment,the levels of interleukin-17(IL-17),hypersensitive C-reactive protein(hs-CRP)and trimethylamine oxide(TMAO)in observation group were lower than those in control group(P＜0.05).After treatment,the levels of phenylalanine,lysophosphatidyllecithin and pyroglutamate in the observation group were lower than those in the control group(P＜0.05).Conclusion The effect of cerebral and intestinal homolo-gy acupuncture in the treatment of acute cerebral infarction is clear,which can reduce the inflammatory response of patients,regu-late the levels of metabolites of intestinal flora and the dysfunction of amino acid and phospholipid metabolism,so as to normalize the disturbed metabolism,promote the recovery of neurological function and improve the prognosis of patients.It has high applica-tion value.
