清瘟败毒饮加减对小儿川崎病(气血两燔证)的临床改善及对患儿血清炎症和免疫功能的影响

目的 观察清瘟败毒饮加减联合丙种球蛋白治疗小儿川崎病(气血两燔证)的临床疗效及其对患儿血清炎症和免疫功能的影响.方法 选择2020年1月—2023年12月于医院心内科治疗的220例小儿川崎病患儿并随机分为两组,每组110例,其中对照组患儿予以阿司匹林联合丙球蛋白治疗,观察组在对照组基础上联合清瘟败毒饮加减治疗.比较两组患儿的临床疗效、症状消失时间,以及治疗前后血清炎症因子水平、免疫功能、心功能变化.结果 对照组患儿的有效率为90.91%(100/110),观察组患儿的有效率为98.18%(108/110),与对照组比较,观察组的有效率显著升高(P<0.05).与对照组比较,观察组患儿的发热等典型临床症状消失时间明显缩短(P<0.01).干预完成后,观察组患儿白介素-6(interleukin-6,IL-6)、C-反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)等血清炎症因子水平相较于对照组均明显降低(P<0.01).干预前两组患儿的免疫功能指标水平无明显差异(P>0.05);干预完成后,观察组患儿CD4+和CD4+/CD8+水平相较于对照组明显升高,而CD8+水平明显下降,差异具有统计学意义(P<0.01,P<0.05);CD3+水平有上调趋势,但差异无统计学意义(P>0.05).干预前两组患儿的心功能无明显差异(P>0.05);干预完成后,观察组患儿血清中心脏型脂肪酸结合蛋白(Heart-type fatty acid binding protein,h-FABP)及B型钠尿肽(B-type natriuretic peptide,BNP)水平相较于对照组均明显下降(P<0.01).干预过程中所有患儿的主要不良反应有恶心呕吐、食欲不振、腹痛腹泻等,症状轻微,两组患儿的不良反应发生率无明显差异(P>0.05).结论 清瘟败毒饮加减联合丙球蛋白治疗小儿川崎病干预效果较好,对改善患儿临床症状、抑制炎症反应、保护心脏功能、调节免疫功能、不良反应有较好干预效果.
Effect of Modified Qingwen Baidu Drink(清瘟败毒饮加减)on Clinical Improvement in Pediatric Kawasaki Disease(Dual Blaze of Qi and Blood Syndrome)and Its Effects on Serum Inflammation and Immune Function in Affected Children
Objective To observe the clinical efficacy of Modified Qingwen Baidu Drink(清瘟败毒饮加减)combined with immunoglobulin in the treatment of pediatric Kawasaki disease(dual blaze of Qi and blood syndrome)and its impact on serum in-flammation and immune function in children.Methods A total of 220 pediatric patients with Kawasaki disease who were treated in the cardiology department of the hospital from January 2020 to December 2023 were selected and randomly divided into two groups,with 110 cases in each group.The control group received aspirin combined with immunoglobulin treatment,while the ob-servation group received Modified Qingwen Baidu Drink in addition to the treatment of the control group.The clinical efficacy,symptom disappearance time,serum inflammatory cytokine levels,immune function and cardiac function changes between two groups of patients before and after treatment were compared.Results The effective rate of the control group was 90.91%(100/110),while that of the observation group was 98.18%(108/110).Compared with that of the control group,the effective rate of the observation group was significantly increased(P<0.05).Compared with that of the control group,the disappearance time of typical clinical symptoms such as fever in the observation group was significantly shortened(P<0.01).After intervention,in the observation group,the levels of interleukin-6(IL-6),C-reactive protein(CRP)and tumor necrosis factor-α(TNF-α)were significantly reduced compared to those of the control group(P<0.01).There was no significant difference in the levels of im-mune function indicators between the two groups before intervention(P>0.05).After intervention,the levels of CD4+and CD4+/CD8+in the observation group were significantly increased compared to those of the control group,while the levels of CD8+were significantly decreased with statistical significance(P<0.01,P<0.05).The level of CD3+showed an upward trend,but the difference was not statistically significant(P>0.05).There was no significant difference in heart function between the two groups before intervention(P>0.05).After intervention,the serum levels of heart-type fatty acid binding protein(h-FAB)and B-type natriuretic peptide(BNP)in the observation group were significantly reduced compared to those of the control group(P<0.01).The main adverse reactions of all patients during the intervention process were nausea,vomiting,loss of appetite,ab-dominal pain and diarrhea,with mild symptoms.There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combined treatment of Qingwen Baidu Drink with modifications and intravenous im-munoglobulin demonstrates considerable efficacy in treating Kawasaki disease in children.This approach notably improves clini-cal symptoms,suppresses inflammatory responses,protects cardiac function,modulates immune function and minimizes adverse re-actions.
