基于中医目诊理论和白睛成像AI及光学技术分析高血压病患者目络特征及发病机制的关联性研究

目的 基于中医目诊理论和白睛成像AI及光学技术分析高血压病患者的目络特征及发病机制的关联性研究。方法 纳入2021年9月1日-2023年9月1日中国中医科学院广安门医院门诊及住院部接受检查的受试者300例,其中高血压病患者(HTN)组172例,血压水平正常患者(NC)组128例。两组别均接受白睛无影成像智能分析系统对白睛眼象进行高清采集、特征提取,然后通过智能分析,最后采用SPSS 26。0进行统计学处理,分析高血压病患者与血压水平正常受试者目络特征形态、目络血脉颜色等特征计数差异以及球结膜血管分区后的计数差异。结果 两组间的"血脉""斑""雾漫"特征分值具有显著性差异。两组间比较,HTN组目络形态"斑"特征分值(18。00±7。07)高于NC组(14。32±5。60)分(P<0。05);HTN组目络形态"血脉"特征分值(30。15±1。15)分高于NC组(26。70±2。27)分(P<0。01)。两组间的"E"(心)"M"(肝)目络区域特征分值具有显著性差异,HTN组白睛络脉区位分值显著高于对照组,"E"区(P<0。05),"M"区(P<0。01)。两组间的"黯红色""黄色""褐色"特征分值具有显著性差异。HTN组白睛络脉颜色"黯红色"(P<0。01),"黄色"(P<0。01)"褐色"(P<0。05)分值显著高于NC组。结论 白睛络脉形态特征、白睛络脉颜色以及球结膜血管分区与高血压病相关联,为中医望目辨证的客观化、精准化提供参考依据。
Analyzing Correlation between Characteristics and Pathogenesis of Eye Collateral in Hypertensive Patients Based on Theory of Eye Diagnosis in Traditional Chinese Medicine and White Eye Imaging AI and Optical Technology
Objective To analyze the correlation study of the characteristics and pathogenesis of the eye collateral in hyperten-sive patients based on the theory of eye diagnosis in traditional Chinese medicine and the white eye imaging AI and optical tech-nology.Methods A total of 300 subjects who were examined in the outpatient and inpatient departments of Guang'anmen Hospital of China Academy of Traditional Chinese Medicine(CATCM)from September 1,2021 to September 1,2023 were enrolled,inclu-ding 172 cases in the hypertension group(HTN),and 128 cases with normal blood pressure levels in the normal control group(NC).Both groups received"MyEyeD-10 White Eyes Shadowless Imaging Health Intelligence Analysis System",also known as the eye diagnostic instrument,which performed high-definition acquisition and feature extraction of the white eye images,fol-lowed by intelligent analysis,and then SPSS 26.0 was used for the statistical processing to analyze the characteristics of the eye collateral of the hypertensive patients and the subjects with normal blood pressure levels.Differences in the counts of features such as morphology,the color of the eye collateral blood vessels and the counts of the bulbar conjunctival blood vessels after parti-tioning were analyzed.Results There were significant differences in the scores of"blood vessels","spot"and"foggy"features between the two groups.In the comparison between the two groups,the score of"spot"feature in the HTN group[(18.00±7.07)points]was higher than that in the NC group[(14.32±5.60)points](P<0.05).The score of"blood vessels"feature in the HTN group[(30.15±1.15)points]was higher than that in the NC group[(30.15±1.15)points]and the score of"blood vessels"feature in the HTN group was higher than that in the NC group[(26.70±2.27)points](P<0.01).There was a significant difference in the"E"(heart)and"M"(liver)eye collateral scores between the two groups,with the white eye col-lateral scores of the HTN group being significantly higher than those of the control group[the"E"region(P<0.05),"M"area(P<0.01)].The scores of"dull red","yellow"and"brown"features were significantly different between the two groups,and the color features of"dull red","yellow"and"brown"of white eye collateral in the two groups were significant(P<0.05).The scores of"dull red"(P<0.01),"yellow"(P<0.01)and"brown"were significantly higher in the HTN group than those in the NC group(P<0.05).Conclusion Morphological characteristics of the white eye collateral,the color of the white eye collateral and the bulbar conjunctival vascular zoning were associated with hypertension,which provides a reference basis for the objectivity and precision of traditional Chinese medicine syndrome differentiation for ocular diagnosis.
