摘要
目的 基于数据挖掘方法分析中药治疗淋巴瘤用药规律,通过总结用药频次、剂量、性味归经及药物间潜在关联和配伍关系等,为临床治疗淋巴瘤组方用药提供参考.方法 本文通过检索中国知网(CNKI)、万方数据知识服务平台(Wanfang Data)、维普中文期刊数据服务平台(VIP)从建库至2023年11月运用口服中药复方治疗淋巴瘤的文献,将符合标准的数据录入 Excel 2019,运用古今医案云平台(V2.3.7)、SPSS Modeler 18.0、Origin pro 2021、Gephi V0.9.2,从用药频次、用药剂量、性味归经、药物关联规则、聚类分析、复杂网络分析角度对文献数据进行统计分析.结果 最终筛选得到符合标准的文献共172篇,整理并录入中药复方307首,涉及中药共381味,总用药频次4 897次.使用频次最高的前10味中药分别是甘草、黄芪、白术、茯苓、半夏、白花蛇舌草、地黄、当归、夏枯草、浙贝母.四气主要为寒、温、平,五味主要为甘、苦、辛,归经主要为肝经、脾经、肺经.关联规则共得到2味药对组合共7组,3味药组合共33组,4味药组合共11组.聚类分析得到6组聚类中药组合.复杂网络分析获得由9个关联性较强中药组成的核心药物组合.结论 淋巴瘤患者以肺脾肾不足为本,气滞、痰凝、瘀血、毒邪为标,病位主要在肺、肝、脾、肾,以扶正祛邪、行气健脾、调和气血、化痰散结、化瘀消瘤、清热解毒为法,处方以六君子汤、当归补血汤、四逆散、二陈汤、白术茯苓汤、半夏白术汤、消瘰丸加减.
Abstract
Objective To analyze the medication rules in the treatment of lymphoma with traditional Chinese medicine(TCM)based on data mining technique.Medication frequency,dosage,nature,flavor,meridian tropism,and the potential association and compatibility among drugs were summarized to provide references for clinical treatment of lymphoma.Methods Literature on the treatment of lymphoma with oral TCM compounds was retrieved from CNKI,Wanfang Data,and VIP from database establishment to November 2023.The data meeting the inclusion criteria were input into Excel 2019.Analysis tools such as Ancient and Modern Medical Cases Cloud Platform(V2.3.7),SPSS Modeler 18.0,Origin Pro 2021,and Gephi V0.9.2 were uesd for statistics and analysis of the collected literature from perspectives of medication frequency,dosage,nature,flavor,meridian tropism,drug association rule,cluster analysis,and complex network analysis.Results A total of 172 eligible articles were selected.307 TCM compounds involving 381 different herbs and medication frequency of 4 897 times were summarized and input.The top 10 most frequently used herbs were Licorice,Astragalus,Atractylodes macrocephala,Poria,Pinellia,Hedyotis diffusa,Rehmannia,Angelica,Prunella vulgaris,and Fritillaria thunbergii.The primary natures of the herbs were cold,warm,and neutral,while the main flavors were sweet,bitter,and pungent.The primarily meridian tropisms were liver,spleen,and lung meridians.The association rule analysis showed 7 groups of two-herb combination,33 groups of three-herb combination,and 11 groups of four-herb combination.Cluster analysis identified 6 groups of herb combination.Complex network analysis revealed a core combination consisting of 9 highly associated herbs.Conclusion The deficiency of lung,spleen,and kidney in lymphoma patients is the root pathogenesis,while the qi stagnation,phlegm coagulation,blood stasis,and toxin are the branch.The main disease locations are lung,liver,spleen,and kidney.Therapeutic strategies include reinforcing healthy qi to eliminate pathogenic factors,promoting qi and invigorating spleen,regulating qi and blood,dissipating phlegm and resolving mass,removing blood stasis and dissipating tumor,and clearing heat and detoxification.Prescriptions such as Liujunzi Decoction,Danggui Buxue Decoction,Sini San,Erchen Decoction,Baizhu Fuling Decoction,Banxia Baizhu Decoction,and XiaoLuo Pill with modifications are recommended.
