槐耳颗粒联合TACE治疗原发性肝癌的Meta分析

摘要
目的 系统评价槐耳颗粒联合TACE治疗原发性肝癌的疗效与安全性.方法 计算机检索CNKI、VIP、CBM、WanFang Data、PubMed和EMbase数据库,搜集 TACE联合或不联合槐耳颗粒治疗原发性肝癌的随机对照临床试验,检索时限均从建库至2024年5月.由2名研究者独立筛选文献、提取数据并评价纳入研究的偏倚风险后,采用RevMan 5.4进行Meta分析.结果 最终共纳入14篇文献,967例患者,其中槐耳颗粒联合TACE组(治疗组)486例,TACE组(对照组)481例.结果 显示治疗组客观缓解率[RR=1.34,95%CI(1.17,1.54),P＜0.000 1]、疾病控制率[RR=1.10,95%CI(1.02,1.19),P=0.02]、一年生存率[RR=1.22,95%CI(1.07,1.39),P=0.004]均高于对照组;治疗组较对照组可以降低胃肠道反应发生率[RR=0.70,95%CI(0.53,0.93),P=0.01]及骨髓抑制率[RR=0.44,95%CI(0.30,0.65),P＜0.000 1].结论 槐耳颗粒联合TACE治疗能提高原发性肝癌的客观缓解率及疾病控制率,延长生存时间,提高患者的生存质量.
Abstract
Objective To systematically evaluate the efficacy and safety of Huaier Granules combined with TACE in the treatment of primary liver cancer.Methods CNKI,VIP,CBM,WanFang Data,PubMed,and EMbase databases were searched by computer to collect the randomized controlled clinical trials of TACE combined with or without Huaier Granules in the treatment of primary liver cancer from the establishment of the databases to May 2024.RevMan 5.4 was used for Meta-analysis after two researchers independently screened literature,extracted data,and evaluated the risk of bias in included studies.Results A total of 14 literatures with 967 patients were included,including 486 patients in the TACE combined with Huaier Granules group(treatment group)and 481 patients in the TACE group(control group).The results showed that objective response rate[RR=1.34,95%CI(1.17,1.54),P＜0.000 1],disease control rate[RR=1.10,95%CI(1.02,1.19),P=0.02],and 1-year survival rate[RR=1.22,95%CI(1.07,1.39),P=0.004]were higher than the control group.Compared with the control group,the treatment group could reduce the incidence of gastrointestinal reactions[RR=0.70,95%CI(0.53,0.93),P=0.01]and the incidence of bone marrow suppression[RR=0.44,95%CI(0.30,0.65),P＜0.000 1].Conclusion Huaier Granules combined with TACE can improve the objective response rate and disease control rate of primary liver cancer,prolong the survival time,and improve the quality of life of patients.
