摘要
国医大师周岱翰教授认为肺结节应"从郁论治",痰浊瘀血凝滞、阻滞于肺络是肺结节的主要病机.张海波教授继承周岱翰教授"从郁论治"思想,采用针药结合治疗肺结节,提出中医治疗肺结节应首重阴阳、寒热辨证,借助舌脉及症状信息辨明其基本属性.治疗上以调神为先,配合健脾益肾补肺、通络散结之法.基于肺结节病位在肺络,位于半表半里之间,常应用针灸疏通经络,借用针药结合手段提高治疗效果,最终达到化痰散结、祛瘀通络的治疗目标.本文阐述针药结合治疗肺结节的理论,总结针药结合治疗肺结节的临证经验,并附验案以举隅.
Abstract
Professor ZHOU Dai-han,a master of Chinese medicine,believes that pulmonary nodules should be"treated from depression".The major pathogenesis of pulmonary nodules is the stagnation of phlegm and blood stasis in the lung meridian.Professor ZHANG Hai-bo inherits Professor ZHOU's idea of combining acupuncture and medicine in the treatment of pulmonary nodules.When treating pulmonary nodules,Professor ZHANG prioritizes the differentiation of yin and yang,cold and heat,then identifies the basic attribute by combining tongue diagnosis,pulse diagnosis,and symptom information.In the treatment,the first priority is to regulate the mind,combined with the methods of tonifying the spleen,kidney,and lung,dredging collateral and dissipating mass.As pulmonary nodules are located in the lung meridian,between half-exterior and half-interior,acupuncture and moxibustion are often used to dredge meridian,and the combination of acupuncture and medicine is used to improve the therapeutic effect,finally achieving the treatment goal of resolving phlegm and dissipating mass,removing blood stasis and dredging collateral.This article elaborates the theory of combining acupuncture and medicine for the treatment of pulmonary nodules,summarizes the clinical experience,and provides effective cases for reference.
