摘要
目的 基于文献挖掘分析中药治疗紫杉烷类药物所致周围神经病变(taxane-induced peripheral neuropathy,TIPN)的中医用药规律.方法 检索中国知网(CNKI)、万方数据知识服务平台(WanFang)、重庆维普中文期刊数据服务平台(VIP)、中国生物医学文献服务系统(SinoMed)从建库至2024年8月的中药治疗TIPN的随机对照试验的相关文献.先用Excel 2021建立数据库,再用SPSS Modeler 18.0、SPSS Statistics 27.0对纳入药物的频次、功效分类、性味归经、复杂网络、关联规则及聚类进行统计与分析.结果 最终筛选出60篇文献,64个处方.纳入的35个内服方中,使用频次最多的5种中药为黄芪、甘草、当归、川芎、党参;29个外用方中,使用频次最多的5种中药是桂枝、红花、川芎、当归、黄芪.内服方和外用方的四气均以温、平药为主;药味均以苦、甘、辛为主;药物多以肝、脾经为主.内服、外用中药治疗TIPN均注重补虚药、活血化瘀药、清热解毒药以通补兼施,内服方偏于理气,而外用方偏于解表、化湿、祛风.结论 中药治疗TIPN多立足该病本虚标实的基本病机,内服以健脾为主,气旺方可行血;外用以活血为主,温通兼顾化湿;同时通络不忘解毒,补虚亦当泻实.
Abstract
Objective To analyze the medication rules of traditional Chinese medicine(TCM)in the treatment of taxane-induced peripheral neuropathy(TIPN)based on literature mining.Methods Literature related to randomized controlled trials of TCM treatment for TIPN was retrieved from CNKI,Wanfang Data,VIP,and SinoMed from database establishment to August 2024.Excel 2021 was used to establish the database.SPSS Modeler 18.0 and SPSS Statistics 27.0 were used for the statistics and analysis of medication frequency,efficacy classification,nature,flavor,meridian tropism,complex network,association rule,and clustering.Results 60 papers and 64 prescriptions were finally screened.Among the included 35 prescriptions for oral administration,the top 5 most frequently used TCM were Radix Astragali,Radix Glycyrrhizae,Radix Angelicae Sinensis,Rhizoma Chuanxiong,and Radix Codonopsis.Among the included 29 prescriptions for external application,the top 5 most frequently used TCM were Ramulus Cinnamomi,Flos Carthami,Rhizoma Chuanxiong,Radix Angelicae Sinensis,and Radix Astragali.The four natures of prescriptions for both oral administration and external application were mainly warm and flat;the flavors based on bitter,sweet,and pungent;and most of medicine based on liver and spleen meridians.TCM for both oral administration and external application in the treatment of TIPN focused on tonifying the deficiency,activating blood circulation and removing blood stasis,and clearing heat and removing toxin to combine both obstruction-removing and tonifying method.The oral administration prescriptions favored the regulation of qi while the external application prescriptions favored the release of exterior,elimination of dampness,and dispelling of wind.Conclusion TCM treatment for TIPN is based on the basic mechanism of deficiency in nature and excess in superficiality.Oral administration medicine mainly focuses on strengthening the spleen,and abundant qi can promote blood circulation.External application medicine mainly focuses on activating blood,give consideration to both warmly dredging method and dampness dissipating.At the same time,do not forget to eliminate the toxin while dredging channel,and supplementing the deficiency should also be accompanied by releasing substance.
