Based on HUANG Yuanyu' theory of the ascending and descending of middle qi,Professor ZHANG Haibo proposes that the pathogenesis connotation of cancer-related insomnia involves the deficiency of middle qi,the impairment of nutritive and defensive levels,and the obstruction of ascending and descending processes and four elements.The subsequent pathological evolution involves liver and spleen's failure to ascend,causing the restlessness of spirit,lung and stomach's inability to descend,leading to the non-retraction of corporeal soul,and non-interaction of heart and kidney,which disturbs spirit and consciousness.Regarding treatment,it is essential to focus on establishing middle qi to pivot middle axis,and in clinical practice Huangya Decoction is used as the basic formula for treatment.The method of regulating four elements should be used to restore the ascending and descending functions,along with methods to soothe liver and gallbladder,relieve depression,and regulate qi to restore the left ascension.Chaihu formulas are often selected.To lower lung and stomach qi,and circulate liver and lung to restore the right descent.Fuling formulas are often selected.To keep coordination between heart and kidney,and adjust both cold and heat.Formulas with Guizhi and mineral substances are commonly selected.This allows the harmonious circulation of qi activity of four elements,balances clear and turbid,and ensures the harmony of ascent and descent,thus ensuring a peaceful sleep.Two proven cases are presented for reference.
