摘要
中医药在治疗老年食管癌中有独特的优势.本文探讨了温阳散结法治疗老年食管癌的理论依据,认为食管癌属有形之实体,因阳虚气化不足、痰瘀积于食络而成."阳化气"不足与肝脾肾三脏阳虚密切相关,脏腑功能失调使得痰饮、水湿、瘀血等阴邪成形太过;清阳不升、浊阴不降致使阴火内生,使瘤体不断增长.治疗上以温阳法贯穿治疗全程,临证辅以化痰散结、行气散结、活血散结、解毒散结等法,以期为临床中医药治疗老年食管癌拓展思路与方法.
Abstract
Traditional Chinese medicine has unique advantages in the treatment of elderly esophageal cancer.This paper discussed the theoretical basis of warming yang and dispersing nodule method in the treatment of elderly esophageal cancer.It is believed that esophageal cancer is a tangible entity,which is caused by the deficiency of yang and qi transformation,and accumulation of phlegm and stasis in the oesophagus.The insufficiency of "yang transforming into qi" is closely associated with deficiency of liver,spleen,and kidney.Dysfunction in these zang-fu organs leads to excessive formation of yin evils such as phlegm,dampness,and blood stasis.The failure of lucid yang qi to rise and failure of turbid yin to descend results in the internal growth of yin fire,causing tumor to grow.The treatment strategy mainly uses the warming yang method throughout the course,supplemented by adjunctive approaches such as phlegm resolution and nodule dispersal,qi moving and nodule dispersion,blood circulation enhancement and nodule elimination,as well as detoxification and nodule clearance,in order to expand the ideas and methods for clinical Chinese medicine treatment of elderly esophageal cancer.
