摘要
中成药临床实践指南(简称"中成药指南")临床问题的确定对此后指南制定过程中的证据检索、证据质量评价和推荐意见形成等具有重要影响,为了体现中成药和疾病阶段在中成药指南制定过程中的重要性,提出具有中医药特色的中成药指南临床问题确定的方法学建议.基于中成药品种(drug,D)、特定疾病阶段(specific disease stage,S)、对照(comparison,C)和特定结局(specific outcome,0)四个方面,提出以DSCO框架为基础,通过科学研究、政策/标准性文件和临床数据补充等收集多来源信息,设置清晰的临床问题遴选标准,最终形成指南拟解决的临床问题.同时为了保证指南的清晰性和完整性,还需将临床问题的收集及遴选过程公开透明报告.
Abstract
The identification of clinical questions for clinical practice guidelines of Chinese patent medicine(CPM)is important for subsequent evidence retrieval,evaluation of evidence quality,formation of recommendations.This paper described a methodological proposal for the identification of clinical questions for CPM guidelines to high-light the characteristics of Chinese patent medicine and reflect its effect in specific stage of the disease.Considering four aspects,namely,the drug of Chinese patent medicine(D),the specific disease stage(S),comparison(C),and specific outcome(0),DSCO framework has been proposed to formulate the clinical questions.Multi-source infor-mation through scientific research,policy or standard documents,and clinical data are suggested for collecting clini-cal questions,and clear selection criteria should be set to finalize the clinical questions to be addressed by the guideline.In addition,the above process needs to be transparently and publicly reported in order to ensure the clarity and completeness of the guidelines.
