摘要
心力衰竭合并肺部感染以心肺气虚、脾肾阳虚、气阴两虚为本,肺热、痰热、瘀血等邪气痼结为标,病性属正虚邪实、虚实夹杂,基本病机为气阴两虚、肺热痰瘀.据此提出心力衰竭合并肺部感染主要治法为清、宣、补、化法,"清"即根据邪热郁闭程度不同,分别予清热解表法、清营透热法、清热凉血法;"宣"主要为宣肺通腑法;"补"以甘温除热法、滋阴益气法、温阳化气法为主;"化"即根据不同病理产物分别予化痰祛湿法、活血化瘀法.
Abstract
Heart failure combined with pulmonary infection is characterized by heart-lung qi deficiency,spleen-kidney yang deficiency,and deficiency of both qi and yin as the the root of disease;lung heat,phlegm heat,and blood stasis as the manifestation of disease.Thus,clearing,dispersing,supplementing,and transforming as the main treatment methods were proposed."Clearing"refers to the method of clearing heat to release the exterior,clearing heat from the ying level to relieve heat,and clearing heat and cooling blood,based on the degree of differentiation of pathogenic heat and stagnation of qi and blood;"dispersing"mainly refers to the method of diffusing the lung and dredging the bowels;"supplementing"refers to the method of relieving fever with sweet-warm medicinals,nourishing yin and tonifying qi,and warming yang and transforming qi;"transforming"refers to the method of resolving phlegm and dampness,invigorating blood and dissolving stasis according to different pathological factors.
