总结朱宗元教授运用通治方合剂治疗颈胃综合征的临证经验.颈胃综合征同时具有颈椎病症状及胃肠道症状,其病位在颈、胃,病机以颈项经络痹阻为本,胃失和降为标.临证主张以经络学说为指导循经论治颈椎病颈肩疼痛及活动受限、肢体麻木、头晕等症状,自拟颈椎通痹方以通络解痉;胃肠道症状多见胃失和降兼寒热错杂之胃脘胀痛、恶心呕吐、嗳气反酸等,常用半夏泻心汤以辛开苦降、寒热并调、和胃降逆.在此基础上,强调颈胃综合征应标本兼顾、颈胃合治,将颈椎通痹方、半夏泻心汤合方化裁而成通治方合剂,并提倡小剂量用药、活用药组以发挥协同增效或减毒之用.
ZHU Zongyuan's Experience in Treating Neck-Stomach Syndrome with Tongzhi Mixture(通治方合剂)
To summarize Professor ZHU Zongyuan's clinical experience in treating neck-stomach syndrome with Tongzhi Mixture(通治方合剂).Neck-stomach syndrome is a group of clinical syndromes with both symptoms of cervi-cal spondylosis and gastrointestinal symptoms.The disease is located in the neck and stomach,with channels and col-laterals obstruction of neck as the root cause and stomach failing to harmonize and descend as the manifestation.It is advocated that the treatment of cervical spondylosis symptoms could be guided by meridian theory in clinic,with self-prescribed Jingzhui Tongbi Formula(颈痹方)to unblock the collaterals and relieve convulsion;gastrointestinal symptoms are mostly seen in stomach failing to harmonize and descend and cold-heat in complexity of gastric disease,commonly used Banxia Xiexin Decoction(半夏泻心汤)with the idea of acrid medicinals open and bitter medicinals direct things downward,regulating cold and heat together,and harmonizing the stomach and directing counterflow qi downward.On this basis,it is stressed that neck-stomach syndrome should treat the root and manifestation simul-taneously,treat neck and stomach together,combine Jingzhui Tongbi Formula with Banxia Xiexin Decoction into Tongzhi Mixture,and advocate using small doses of herbs,applying herb-pairs flexibly,in order to play a synergistic effect or reduce the toxicity of medications.
