摘要
基于中医五体痹、五脏痹理论,结合风湿免疫病多系统受累、传变复杂多变的特点,在传承名老中医治痹学术思想、总结临床实践经验的基础上,提出风湿免疫病"体脏合痹"理论.认为体痹非独存在于皮、肉、筋、脉、骨,而可能发生于在外的所有体表组织;脏痹亦非独存在于肝、心、脾、肺、肾,还可能发生于包括六腑、奇恒之腑在内的所有脏器.脏居于内,藏精以养体,体居于外,乃脏的外延;体与脏通过五行生化统属、经络络属相贯、气血津液环流、精神脑髓主宰,构成了内外相合的系统网络,形成了体痹及脏、脏痹及体、体脏合痹的疾病传变规律.提出"和正调衡"的总体治疗原则和"以体养脏、调脏安体、体脏同治"的诊疗思路,为风湿免疫病的临床防治提供方法.
Abstract
Based on the theories of five body constituents bi(痹)and five zang(脏)organs bi,considering the characteristics of rheumatic and immune diseases involving multi-system damage and complex progression,and by in-heriting the academic ideas of famous TCM practitioners in treating bi and summarizing their clinical experience,the theory of"combined bi of body constituents and zang organs"has been advocated.It is believed that body constituents bi does not only exist in the skin,flesh,tendons,vessles,and bones,but may occur in all external body tissues.Zang organs bi does not only exist in the liver,heart,spleen,lungs,and kidneys,but may also occur in all organs including the six fu(腑)organs,the extraordinary fu organ.Zang organs are located inside,storing essence to nourish the body,while the body consituents are located outside,which are the extensions of the zang organs.Body constituents and zang organs are closely connected through the generation and transformation of the five phases,the interweaving of meridians and collaterals,the circulation of qi,blood and fluids,and the domination of the spirit and brain marrow,forming a systematic network that matches the inside and the outside,as well as the disease progression laws involving body constituents bi affecting the zang organs,zang organs bi affecting the body constituents,and com-binedbiof body constituents and zang organs.Thus,it is advocated that the overall treatment principle is"harmoniz-ing balance between healthy qi and pathogenic qi",and for the diagnosis and treatment ideas,it should give full play to strengthen body constituents to nourish the zang organs,regulate the zang organs to nourish and heal the body con-stituents,and treat the body constituents and zang organs simultaneously,all of which provide methods for the clinical prevention and treatment of rheumatic and immune diseases.
