血小板参数与血液透析相关性头痛

摘要
目的 探讨血小板参数及中性粒细胞与淋巴细胞比值(Neutrophil to lymphocyte ratio,NLR)与血液透析相关性头痛(Hemodialysis-related headache,HRH)的关系.方法 选取2022年10月 2022年12月在武汉大学人民医院接受血液透析治疗的127例患者,根据国际头痛第3版分类诊断标准分为HRH组(n=55)和对照(comparative control,CC,n=72)组;比较 2 组基线资料;视觉模拟评分(Visual analogue score,VAS)、头痛影响测试(Headache impact test,HIT-6)、抑郁自评量表(Self-rating depression scale,SDS)和焦虑自评量表(Self-rating anxiety scale,SAS)分别用来评估HRH的严重程度、头痛患者的生活质量以及抑郁焦虑程度.结果 与CC组比较,HRH组血小板计数(Platelet counts,PLT)、血小板比积(Platelet hematocrit,PCT)较低,平均血小板体积(Mean platelet volume,MPV)、血小板体积分布密度(Platelet distribution width,PDW)、大血小板比率(Platelet large cell ratio,P-LCR)以及 MPV/PLT 较高(P＜0.05);多变量 Logistic 回归分析显示MPV,PDW,PCT以及P-LCR是HRH的危险因素(P＜0.05);受试者工作特性曲线(Receiver op-erating characteristic curve,ROC)显示MPV/PLT诊断HRH的灵敏度和特异性最高;HRH患者的MPV/PLT 水平与 VAS 和 HIT-6 评分呈正相关(r≥0.308,P＜0.05).结论 PLT,PCT,MPV 以及 MPV/PLT 是HRH的独立危险因素,MPV/PLT水平与HRH的严重程度和生活质量呈正相关.
Abstract
Objective To investigate the relationship between platelet parameters,the neutrophil-to-lymphocyte ratio(NLR)and hemodialysis-related headache(HRH).Methods A total of 127 patients receiv-ing hemodialysis at the Renmin Hospital of Wuhan University from October to December 2022 were included.They were divided into a hemodialysis-related headache group(HRH group,n=55)and a comparative control group(CC group,n=72)based on the criteria of the 3rd edition of the International Classification of Headache Disorders(ICHD-3).Compared and analyzed the baseline data of the two groups.The visual analog scale(VAS),headache impact test(HIT-6),self-rating depression scale(SDS),and self-rating anxiety scale(SAS)were used to evaluate the severity,the quality of life,and the degree of depression and anxiety of the HRH patients.Results Compared to the CC group,the HRH group had lower platelet count(PLT)and platelet percentage(PCT),but higher average platelet volume(MPV),platelet volume distribution width(PDW),and large platelet ratio(P-LCR)(P＜0.05).Multivariable Logistic regression analysis showed that MPV,PDW,PCT,and P-LCR were risk factors for HRH(P＜0.05).The ROC curve showed that MPV/PLT had the highest sensitivity and specificity for diagnosing HDH.The MPV/PLT level of HDH patients was positively related to the VAS and HIT-6 scores(P＜0.05).Conclusion PLT,PCT,MPV,and MPV/PLT were independent risk factors for HRH.The level of MPV/PLT was positively related to the severity and the quality of life of HRH.
