    Development and Validation of Diagnostic Criteria for Elderly Atopic Dermatitis

    查看更多>>摘要:Objective:Elderly atopic dermatitis(AD)is a newly identified subtype of AD.Whether specific diagnostic criteria are needed for elderly AD has been debated.This study aimed to propose diagnostic criteria for elderly AD and evaluate the sensitivity.Methods:A hospital-based study was conducted.We screened the clinical features of 1,312 patients with AD of different ages in 1 cohort and proposed a set of diagnostic criteria for elderly AD.The criteria were then validated in another cohort of 223 patients clinically diagnosed with elderly AD by dermatologists specialized in AD to examine the diagnostic sensitivity compared with other criteria by chi-square test.Results:Based on the patients'clinical features,a set of diagnostic criteria for elderly AD were proposed.The new diagnostic criteria showed significantly higher sensitivity than the classical diagnostic criteria(P<0.001),especially for mild and moderate AD(P<0.001).Of all 223 patients with elderly AD,93.3%fulfilled our criteria,while only 43.5%,65.5%,and 52.0%fulfilled the Hanifin and Rajka criteria,the Japanese Dermatology Academy criteria,and the United Kingdom Working Party criteria,respectively.Conclusion:The newly proposed criteria for elderly AD yielded high diagnostic sensitivity,particularly for mild and moderate AD.

    Serum 25-Hydroxyvitamin D Level Might Be a Useful Indicator of Disease Severity in Obese Children With Atopic Dermatitis:A Case-Control Study

    查看更多>>摘要:Objective:To assess the relationship between the severity of atopic dermatitis(AD)in children of varying weight categories and their serum 25-hydroxyvitamin D[25(OH)D]levels.Methods:The study population comprised 899 patients with AD and 854 age-and sex-matched controls.The Mann-Whitney U test and Kruskal-Wallis H tests were used to assess differences between groups,and Spearman correlation analysis was used to test correlation.Results:The 25(OH)D level in the AD group was M(Q25,Q75),24.0(19.7,28.4)ng/mL,which was significantly lower than the control group(26.4[23.6,29.9]ng/mL;Z=-3.34,P=0.001).25(OH)D levels in children with AD were negatively correlated with body mass index(r=-0.30,P<0.001),Severity scoring of Atopic Dermatitis(SCORAD;r=-0.14,P<0.001),total immunoglobulin E(r=-0.13,P<0.001),and eosinophil(r=-0.08,P=0.017).There were statistically significant differences in 25(OH)D(H=18.46,P<0.001),total immunoglobulin E(H=9.13,P=0.010),eosinophil(H=67.17,P<0.001),and SCORAD(H=10.49,P=0.005)among groups with different body mass index classification.The 25(OH)D levels were 22.3(17.5,27.1)ng/mL in the overweight AD group and 22.3(17.6,25.7)ng/mL in the obese AD group,which were significantly lower than those in the normal-weight AD group(24.7[20.4,25.5]ng/mL;P=0.003,P=0.004).25(OH)D levels were negatively correlated with SCORAD in obese AD patients(r=-0.25,P=0.010).Conclusion:Vitamin D insufficient or deficient is obvious in children AD patients.The 25(OH)D levels in the overweight/obese AD group are significantly lower than those in the normal-weight AD group.Vitamin D level is negatively correlated with SCORAD in obese children with AD.

    Cutibacterium acnes in Atopic Dermatitis:Roles and Potential Therapeutic Applications

    查看更多>>摘要:Atopic dermatitis(AD)is a chronic relapsing inflammatory skin disease.The main initiating mechanisms of AD are disruptions in the skin microbiota,dysfunction of the skin barrier,and predominantly elevated type 2 immune responses.Cutibacterium acnes(C.acnes)is a commensal bacterium that is ubiquitous and predominant in healthy skin and shows intraspecific subtype diversity.The abundance of C.acnes is closely related to the sebum secreted by sebaceous glands.C.acnes has long been considered a proinflammatory skin bacterium that drives the development of acne vulgaris.Growing evidence indicates that C.acnes promotes skin microbiota homeostasis and skin barrier maintenance;however,the potential role of C.acnes in AD remains largely unexamined.This review provides the latest information on the distribution of C.acnes and its phylotypes in healthy skin and AD as well as an overview of the possible role of C.acnes in the pathophysiological pathways underlying AD.Additionally,the review focuses on new evidence regarding the protective functions of C.acnes and its metabolites in AD and discusses the potential for therapeutic applications.

    查看更多>>摘要:The skin constitutes the primary barrier between the human body and the external environment.The skin microbiome plays an important role in the microecosystem,on the skin surface and mediates multiple facets of the skin barrier function,including physical,chemical,microbial,and immune barriers.In this review,we discuss the host-microbiota interactions in barrier maintenance and disruption,implications for skin disorders such as atopic dermatitis,psoriasis,and rosacea,and the latest strategies for targeting the skin microbiota to improve the skin barrier.

    查看更多>>摘要:Objective:Beau lines,onychorrhexis,and psoriatic lesions of the dorsal nail plate may be missed by photographic methods,indicating a need for surface texture measurement methods that are more quantitative,sensitive,and repeatable than visual inspection or imaging.We conducted this study to evaluate the utility of surface texture measurements of cadaveric nails that can be associated with histopathological studies in the future.Methods:The nail plates of 4 cadaveric right pollices and halluces were cleaned and molded,cast in clear epoxy,and scanned with a Sensofar S Neox optical profilometer.A one-way analysis of variance was performed to determine statistical significance of findings.Results:Almost no differences were observed between the pollex and hallux scans and between the distal and proximal regions.The greatest differences were found between individuals.Although lower magnification(5x)is less sensitive than higher magnification(20x),the lower magnification represented and characterized more of the dorsal nail plate.Conclusion:The dorsal nail plate areal roughness has a measurable range of values that may serve as a starting point for evaluating pathological findings,particularly Beau lines and psoriatic lesions.The sensitivity of these techniques may be especially valuable in the recognition of less severe states of these diseases that may enable the diagnosis of earlier stages of growth disruptions(Beau lines)or psoriasis.Such applications could be especially useful in investigations of the health of wildlife populations or vulnerable human populations with incomplete treatment records.

    查看更多>>摘要:Objective:Leprosy is an infectious diseases that remains a concerning public health issue,The infection of Mycobacterium leprae still exists at the county level in hard-to-access areas in southwest China.An accurate analysis of the prevalence of leprosy in various regions is necessary for the formulation of prevention and control strategies,so we conducted this study to describe the epidemiological characteristics of leprosy in China in 2021 and provide essential information for future national leprosy control and prevention strategies.Methods:We collected epidemiological data of leprosy from 31 provinces,autonomous regions,and municipalities of China(except for Hong Kong,Macao,and Taiwan)provided by the Leprosy Management Information System.We performed a comparative analysis of the epidemiological features of patients with leprosy including age,sex,geographical distribution,and grade of disability.Results:In total,374 newly detected leprosy cases were reported nationwide in China in 2021,giving an incidence rate of 0.27 per million population,which represented a decrease of 7.9%compared with 2020.Among the population with leprosy in 2021 that comprised 238 males(63.6%)and 136 females(36.4%),1.9%(7/374)were children younger than 15 years,92.8%(347/374)had multibacillary leprosy,and 18.2%(68/374)had grade 2 disability.More than half of the new leprosy cases(54.8%,205/374)were distributed in southwest China.There were 33 relapsed leprosy cases reported in 2021.By the end of 2021,there were 1,897 registered leprosy cases reported nationwide,giving a prevalence rate of 1.35 per million population.Conclusion:The leprosy epidemic in China shows a downward trend in terms of the prevalence and incidence rates.However,the high prevalence of leprosy in southwest China is still a matter of concern.

    查看更多>>摘要:Objective:Vitiligo is a chronic,acquired depigmentation of the skin,which can be divided into active and stable vitiligo.The stable and active stages are not immutable and often alternate.Identification of internal markers would help to diagnose the active stage of vitiligo,and the purpose of this study was to find potential active markers in the progression of vitiligo.Methods:We compared 30 healthy controls to 60 patients with vitiligo(30 with active disease and 30 with stable disease)regarding the expression of serum monocyte chemoattractant protein(MCP)-1,chemokine(C-C motif)ligand(CCL)3,CCL5,CCL21,interleukin(IL)-6,and IL-17.Specimens were collected from May 2017 to December 2018.Serological detection was carried out by enzyme-linked immunosorbent assay.The difference between groups was compared by Kruskal-Wallis H test and Mann-Whitney U test.Results:We found that the serum levels of MCP-1,CCL3,IL-6,and IL-17 were significantly higher in patients with vitiligo than in healthy controls(P<0.01)and that IL-17 was higher in patients with active vitiligo than in those with stable vitiligo(P<0.01).After 3months of treatment,the serum IL-17 level of patients with active vitiligo was not significantly different from that before treatment(P=0.526);after 6months of treatment,however,the serum IL-17 level had significantly decreased(P<0.01).Conclusion:These findings suggest that the serum IL-17 level can be used as a marker of vitiligo disease activity giving it certain guiding significance for clinical treatment.

    查看更多>>摘要:Vitiligo is an autoimmune disorder caused by the destruction of melanocytes in various body regions.Numerous diseases may accompany vitiligo.The prevalence of vitiligo varies among geographical regions and shows no relation to sex or age.However,vitiligo most commonly affects people aged 10 to 30 years.The clinical symptoms of vitiligo include irregular patches on the skin and various diseases associated with this disorder.The present review focuses on the clinical effects and risk factors of vitiligo-associated conditions and the current therapeutics for treating vitiligo.This systematic review was performed based on the PRISMA guidelines and articles published from 2000 to 2023 in Web of Science,PubMed,PsycINFO,and Embase.The most clinically significant diseases associated with vitiligo are alopecia areata,autoimmune thyroid disease,Addison disease,and type 1 diabetes mellitus.No targeted population is specifically prone to vitiligo;it does not affect any particular race or sex.Hence,awareness of the diseases associated with vitiligo is necessary to control them through various medical and surgical treatments with the proper facilities.

    查看更多>>摘要:Introduction:Epidermolysis bullosa pruriginosa(EBP)is a rare clinical subtype of inherited dystrophic epidermolysis bullosa(DEB)caused by type Ⅶ collagen mutations.The onset of EBP is variable and may present in late adulthood.The clinical features of EBP include prurigo-like papules,plaques,nodules,or linear configuration on the lower extremities.Here,we reported two sisters with EBP.Case presentation:We identified two Thai sisters with mild to moderate form of EBP,which resulted from a shared glycine substitution(Gly2287Val)in COL7A1 identified by genomic sequencing.Discussion:The histology and molecular findings of both cases supported a diagnosis of dystrophic EBP,however,the clinical manifestations differ between both cases.Conclusion:Molecular testing is the key for the diagnosis of EBP due to nonspecific clinical manifestation and histologic findings,however,there is no clear genotype-phenotype correlation in EBP.

    Human Papillomavirus Within Psoriasis Plaques in a Patient With Psoriasis:A Case Report

    查看更多>>摘要:Introduction:Psoriasis may be triggered by several factors,and its pathophysiology is related to inflammatory cellular processes.We herein describe a patient with psoriasis who developed warts on multiple psoriasis plaques and was successfully treated with systemic subcutaneous methotrexate.Case presentation:A 67-year-old man who had been diagnosed with psoriasis 14 years previously developed an outbreak of small erythematous desquamative plaques distributed symmetrically on his arms and trunk.Multiple spiny whitish papules had developed over his previously established psoriasis lesions.The appearance of the papules was consistent with viral warts.The patient was treated with systemic methotrexate injected subcutaneously and oral folic acid,both dosed weekly.This treatment produced good results.Discussion:A remarkable feature of this case is that psoriasis was the first diagnosis and that human papillomavirus(HPV)appeared over widespread erythematous squamous plaques.The pathophysiology of psoriasis is related to inflammatory cellular processes.Some authors have reported that HPV proteins may stimulate keratinocyte proliferation.Others have reported that HPV infection mostly stimulates systemic interleukin 17 production;consequently,HPV infection could provide a better environment for interleukin 17 secretion and facilitate new onset of psoriasis.One report indicated that methotrexate was discontinued in a patient who presented with both pathologies,although we had very good results.Conclusion:Psoriasis and HPV may be pathophysiologically related.HPV may stimulate keratinocyte proliferation and systemic interleukin 17 production,resulting in a better environment for psoriasis.Systemic methotrexate injected subcutaneously resulted in a good outcome in our patient.
