    The Mechanism,Impact,and Effectiveness of Herpes Zoster Vaccine:A Narrative Review

    查看更多>>摘要:In the past decade,there has been increasing interest in whether a widespread childhood varicella immunization scheme may increase the age-specific incidence of herpes zoster in adults.This proposition is based on evidence that exposure to endemic varicella boosts varicella-zoster virus-specific immunity in adults and that cessation of varicella in the community would result in a decline in the T-cell-mediated immunity required to maintain dormancy of varicella-zoster virus in neurons.However,numerous epidemiological studies have failed to fully explore this effect.Quantifying the efficacy of the varicella vaccine is a crucial step in understanding its benefits.We conducted this review to quantify the efficacy of the varicella vaccine(HZ/su vaccine)which will contribute immensely to our understanding of how the vaccine benefits the population in prevention against the herpes zoster,especially during the outbreak of Covid-19.

    Successful Treatment of Generalized Pustular Psoriasis with Adalimumab in a Pediatric Patient:A Case Report

    查看更多>>摘要:Introduction:Generalized pustular psoriasis(GPP)is a rare life-threatening subtype of psoriasis and continues to be a therapeutic challenge.The first line therapies include acitretin,cyclosporin A,and methotrexate which have long-term side effects for children.Adalimumab has a favorable safety profile with low rates of adverse events,and has been described for successful treatment of adults with GPP in a few reports.We herein describe a 3-year-old girl with a body weight of 14.2 kg in whom adalimumab was used to successfully treat GPP.Case presentation:A 3-year-old girl with a 10-day history of systemic symptoms of fever,malaise,and the onset of studded painful pustules overlying an erythematous base on the scalp,face,neck,trunk,and bilateral extremities,affecting a body surface area of 90%.Laboratory examination results were notable for leukocytosis and elevated inflammatory markers.Biopsy confirmed the diagnosis of GPP.After failure of conventional therapies including cyclosporine and methotrexate,the patient began off-label treatment with adalimumab and achieved complete lesion clearance.During 14-month follow-up,she remains in remission on maintenance adalimumab therapy.Discussion:No consensus on the standard treatment regimen for GPP has been reached.Unfortunately,all drugs that have been reported to be effective for GPP are off-label.These treatments include systemic immunomodulators such as cyclosporine and methotrexate,systemic retinoids such as acitretin and isotretinoin,and various biologics.Our findings lend weight to the scant evidence describing the effective use of adalimumab in the treatment of pediatric GPP.Conclusion:Off-label use of adalimumab may be an effective and safe therapeutic option for GPP.

    Eruptive Pseudoangiomatosis and Parvovirus:Exclusive Findings in Clinical Manifestation from a Report of Four Cases

    查看更多>>摘要:Introduction:Eruptive pseudoangiomatosis(EP)is a rare type of cutaneous exanthema characterized by red angioma-like papules.An association of EP with viral infection has been reported.We herein describe 4 pediatric patients who showed a new variant of EP-associated rash with atypical clinical features and positivity for parvovirus B19 infection.Case presentation:Pediatric patients showed a new variant of EP-associated rash with atypical clinical features by dermoscopy and reflectance confocal microscopy.The papules were localized to the face(cheeks,forehead,and eyelids).Dermoscopy showed multiple red dots suggesting dilated capillaries surrounded by reddish structureless areas;and in vivo reflectance confocal microscopy(RCM)showed perivascular inflammation around the dermal papillae.However,characteristic perilesional halo was absent.In all patients,serology revealed evidence of active infection by parvovirus B19.Discussion:Few cases of a facial pseudoangiomatous rash that is related to parvovirus B19 infection and lacks the typical anemic halo have been reported in the literature.Conclusion:These cases provide new information on the clinical spectrum of EP and its association with parvovirus.

    Multiple Cutaneous Nodules as Clinical Harbinger of Gallbladder Adenocarcinoma:A Case Report

    查看更多>>摘要:Introduction:Cutaneous metastasis is defined as invasion of the skin by malignant cells originating elsewhere in the body.Cutaneous metastasis of adenocarcinoma gall bladder is extremely rare.The skin can be involved simultaneously or metachronously with the primary tumor.Here,we reported a case of cutaneous metastases as a clinical harbinger to an otherwise difficult to diagnose primary adenocarcinoma gallbladder.Case presentation:A 46-year-old male presented with multiple nodules on scalp,trunk,and limbs along with fever,weight loss,and lymphadenopathy.Histopathologic examination of the nodule revealed a malignant infiltrating glandular neoplasm in dermis,consistent with metastatic adenocarcinoma with intestinal metaplasia on immunohistochemistry.Radiological investigations further confirmed the primary tumor in gall bladder with metastases in liver and lymph nodes.Unfortunately,the patient succumbed to his disease.Discussion:Gallbladder adenocarcinoma is the most commonly seen tumor of the biliary tract.Skin involvement in malignancy helps determine the treatment course and has inevitable prognostic significance.Conclusion:We reported multiple cutaneous nodules as a remote manifestation of gallbladder adenocarcinoma.Biopsy of the skin lesion was easy to perform and quite reliably guided us toward the diagnosis of the primary tumor which would have been very difficult to diagnose otherwise.

    查看更多>>摘要:Aminolevulinic acid-based photodynamic therapy(ALA-PDT)is a safe and effective physical therapy for acne vulgaris that is widely used in clinical practice.To further standardize the clinical application of ALA-PDT in the treatment of acne,we created this updated consensus based on the 2011 version of the consensus on ALA-PDT for the treatment of acne vulgaris.In this document,we summarize the mechanisms of action of ALA-PDT and provide practical standards for patient selection,treatment parameters,administration procedures,and management of adverse reactions.Our aim was to provide guidance and treatment regimens for using ALA-PDT for acne vulgaris in clinical practice.
